0020682298 2016-06-15 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor is drilling a 12.25 borehole at a depth of 1746. The wellbore is producing 450 barrels per hour from the Big Injun formation. Meyers Well Service has several water trucks on location to assist in handling the large volume of water.
0858798224 2016-06-21 Plug / Plug Back No Violations 06/21/2016 - On site inspection to witness the well plugging process due to the excessive amount of water influx experienced at 1,290 from the Injun Sand formation while drilling the 12.250 1st intermediate well bore. Frontier Drilling rig # 32 is on the hole and has spotted the 5 drill string on bottom to set the bottom plug at 5,080 and consisting of class H cement. The drill string was then pulled up to 1,800 and a gel space set in place to support the Berea plug, once the 100 spacer was set the drill string was pulled to 1,700 and a 206 sack 200 plug was set to cover the Berea Sand formation. The drill string was then pulled up to 600 and the cement allowed to sit for 3 hours before running in and tagging the top of cement.
1465693625 2016-06-11 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Frontier rig 32 drilled 1045 of 17.5 borehole and then installed 1035 of 13.375 casing. Wyoming Casing provided the power tong service. Haliburton cemented the casing in place with 1030 sacks of HalCem cement. I witnessed the cement weight verification by pressurized cement scale. Circulation was maintained and 55 barrels of cement returned to surface. The mix water PH and chlorides were tested before the job.
1499860050 2017-07-11 Final Restoration No Violations The purpose of my inspection today was a status check on a final restoration. During my inspection the location of the well was graded with a gravel surface. The plugged well was located on a well pad with five production wells. Location has met 1509.072(B) Final restoration requirements.
1542653243 2018-11-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 550 pounds per square inch, the 9.625-inch diameter casing had 350 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 12- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site near located on the northwest corner of the pad. Identification tags were present, and production was up to date.
1703678270 2016-06-22 Plug / Plug Back No Violations On site inspection at 1:00 am to witness the setting of a cement plug over the Big Injun Sand and Kenner Formations along with the plug designed to also cover the bottom of the surface casing marked at 1,035. Frontier Drilling rig # 32 is on location and currently 2 plugs have been set in the well bore on 06/21/2016 with the last plug set to cover the Berea Sand formation from 1,620 to 1,630 and after waiting 6 hours for the plug to cure the plug was tagged at 1,520 with the 5 drill string before proceeding. Moving foreword at 1:45 am a 20 barrel 6% gel spacer was spotted at 1,490 through the 5 drill string and the drill string was then pulled up to 1,400 and a 475 sack Class G plug was set and will be allowed to cure before the company attempts to verify the plug is in place and the bottom of the surface casing is covered, off location at 2:30 am. At 8:00 am I arrived back on location and witnessed the plug being tagged at 930. The rig then pulled the 5 drill string up to 506 and set a 364 sack class G surface plug with 10 barrels of good cement returns to surface. Will all cement weights being verified throughout all plugs set by weight of scale. The drilling rig will pull the drill string and break down the surface control equipment (BOP and flowlines), at that point the top of cement will be once again verified and topped off accordingly as needed before installing the permanent marker below grade.
1946714402 2016-06-18 Drill / Deepen / Reopen No Violations On site status check inspection as a response to the company reporting the flow line failed between the well head and shaker screen area. Upon arrival it was observed that the flow line was damaged and folded in 1/2 and laying down on the ground near the backyard area, the rig draw work covers were also damaged. It was reported that the well bore was making excessive amounts of fluid at a depth of 1,290 from the Big Injun sand formation while the company was drilling the 12.250 intermediate well bore below the Injun at a measured depth of 4,804, the company indicated they stopped circulation to make a drill string connection and the water influx was great enough to cause a restriction downhole and when they began to circulate after the connection a pressure surge was experienced when the column of fluid was lifted to surface causing the flow line equipment to fail at surface. The company reported they were circulating with air at 4,800 cubic feet per minute. The company automatically switched over to fluid and maintained circulation with 9 lb/gl brine through the choke manifold on the BOP stack. Frontier Drilling rig 32 is on the hole and will complete the remaining 1,900 of the 12.250 section on fluid. The company also is bringing in drilling fluids to mix a pill to stabilize and condition the well bore and attempt to stop the water influx from the Injun formation. No product was lost off containment and no product was observed present inside the containment area No injuries were reported.
1962444851 2015-10-13 Casing String Witnessed No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services drilled a 32 borehole to a depth of 100. Express then ran 100 of 26 weld casing. Express cemented the casing in place with 330 sacks of class A cement. I witnessed 22 barrels of cement circulate to surface.
2137576168 2016-06-23 Plug / Plug Back No Violations The cement top is 22 from surface. The company will top off the column when they cement the next conductor.
2142235345 2016-06-20 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor has resumed drilling operations and are currently drilling a 12.25 borehole at a depth of 5,028. They are drilling on fluid with a PDC bit.
