1422565138 2018-07-09 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. The location had been graded with vegetation growth. No erosion issues were noticed. The well was equipped with 4.5 inch diameter production casing with a tubing head, 2.375 inch diameter tubing, rods, and a pumping unit powered by natural gas. The production valve on the tubing was in the open position. The line feeding the natural gas motor was connected to the 4.5 inch casing. The production equipment consisted of one 210 barrel tank, one 100 barrel tank, one horizontal separator, and one sales meter. The sales meter had a 2 inch diameter sales line, .375 inch orifice plate, and a chart recorder with a current chart. Northwood Energy has submitted reports of production for the year 2017. The identification was located on the 210 barrel tank. The location has met Preliminary Restoration requiments.
1464719081 2016-05-31 Casing String Witnessed No Violations 05/31/2016 -Decker Drilling rig #9 is rigged up on the hole and drilled a 120 x 15 conductor borehole and set 107 of 11.750 conductor pipe. Formation cementing cemented the conductor using 90 sacks of class A cement with cement returns witnessed to surface and circulation established prior to and maintained throughout the cement job.
1464823101 2016-06-01 Casing String Witnessed No Violations On site inspection to witness the running and cementing of the surface casing. Decker Drilling is on the hole and has drilled a 11 surface borehole to 1,340, the hole was dusted to 740 and misted past that point. 1,280 of 8.625 24LB/FT casing was ran with a guide shoe - 5 centralizers installed along with a cement basket at 300. Formation Cementing was on location to cement the casing using a lead and tail cement mix with 160 sacks of class A-3% calcium -3% gel ran as the lead cement and a class A 3% calcium 2% gel tail cement mix ran. Circulation was established prior to cementing and the cement was displaced with 81 barrels of fresh water with the casing pressure tested to 500 psi over cement displacement lift pressure and cement returns witnessed to surface.
1527761470 2017-07-10 Production Wells No Violations The purpose of my inspection today is a status check on a small oil release within a lined earthen dike surrounding the wellhead. During my inspection today the oil saturated oil absorbent pads have been removed from location and new oil absorbent pads have been placed in the dike. Follow ups will be completed until the oil absorbent pads and contamination have been removed.
