-1255613616 2012-03-30 Inspection of the well head found the following equipment -4.5 production casing -2.375 tubing with all commercial valves closed and a pumping unit in place -At the production fluid storage area 3-210 tanks -3 separators and meters are present in which none of the gas meters have charts present and the burner tube on one of the 210's is leaking ,no well identification is present to indicate which tanks belong to which wells in area. No reported production on said well since 2007.

-754992848 2013-10-09 On the day of my inspection, I found no change in the status of the well from my last inspection, the permit to plug has been issued on 11/30/2012, there is oil dry that has been spread around the tanks where containment ion has occurred, I placed a phone call to Scott Davis with Green Gas on 09/30/2013 to remove the tanks and the containment soil, Green Gas has not complied with the verbal agreement as of this day, I placed a second phone call on this day of 10/09/2013 to Green Gas on there voice mail to return my call as soon as possible.
1505010985 2012-10-24 The day of my inspection I found the divisions production records are not current, the well is not clearly identified, there is a pumping and a drip at the well site. At the road side there are (3) two hundred ten barrel production tanks one is leaking oil/ steel brine tank/ separator/ drips. The well is to be plugged/ produced or placed in temporary inactive status, also properly identify the well. The leaking tank is to be removed and the oil is to be cleaned up as well as the contaminated soil removed, failure to comply will result in forfeiture of bond. 9-9-05(A)10
