| I stopped in location and Chipco was milling on well with a junk mill. They pulled out the bit and it revealed that the bit was milling on a joint of crooked tubing or broke off 4.5''casing. They had ran an impression tool. The company had been at 1600'. They had pulled cork srew tubing and broke of 4.5''casing 8 times. The mill would not go down any farther. They worked and made the attempt for two weeks. Per my discussion with supervisor we both agreed to start plugging. This is a Queenston well. There was no bottom hole plug or lime plug set. I took pictures of the pipe and tubing. There were four 2.875'' collars and 2.375'' tubing and mill to depth of 1669'. Decided to Pull 4.5''casing in morning and see what was retreived.

| On the day of my inspection I seen the valves on the four inch casing were closed, there was no pumping unit at the well site, there is (1) two hundred ten barrel production tank/ separator/ setting inside a containment dike, the I.D. is posted on the production tank, the divisions records for the year of 2010 and 2011 do not meet division standards. The requirements will be to plug or produce or place in temporary inactive status, failure to comply will result in forfeiture of bond.
