-387251395 2009-10-21 Area around well needs repair seeding. Seeding operator completed on site, has washed down slope. Tank battery dike has a drain installed. Drain pipe has a cap on it that is not sufficient to prevent escape of fluids if there is a leak at the battery. Restoration can not be approved at this time. Sent e-mail to Tim Reilly at John D Oil requesting repairs.
-915896064 2013-07-23 My inspection found the well to be equipped with a plunger lift unit. The annulus valve is closed. The well and tank battery are in good repair. The tank battery consists of a short 100 BBL production tank, a short 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1958696845 2009-12-02 Site has been mulched around the well head. Vegetation is becoming established. Tank battery dike is stable. Grading has been completed. There as drainage coming from behind the tank battery off of the slope, the drainage flowed to the north end of the battery around the battery to the front of it, across the front and into a drainage culvert. Drainage has a light sediment load. Sediment didn't appear to be coming off of the dike. Lease road is stable and in good repair. Lease road gate was not installed, drive is primary drive for the landowner.
262776461 2009-02-26 Surface has been set, on site to pick up surface ticket. Site is in good repair. Operator was installing air body and BOP. CND was driven in. Bob Randles called from the company, operator drove in the CND pipe. Rig had a hard time running surface pipe, when tripping surface pipe out CND feel two feet into hole. Cementing of CND was waived due to bedrock being at 20'. Operator had to trip back in and clean hole. Drill String was pulling tight at 140'. During drilling operator did not note any Berea. From reviewing water well records and adjacent wells Berea bottom should have been between 140' and 150'. Bob stated that they didn't lose any circulation, but didn't get cement to return to surface. They did get dirty water back. Operator will need to grout surface during long string.
