-1174478103 2013-04-02 Well equipped with pumping unit with engine but no belt. Well not equipped with an identification. Tank battery not located. The 4 1/2 production casing was open to sales and to a 1 black plastic line with a step down regulator. There was a scald area around the well head roughly 2 x 5', and area along side and down hill of the unit roughly 15 x 12', and a 2 x 2' hole in the ground near the rear of the unit roughly 1-2' deep. Gas and production fluids were bubbling into the hole from below creating a small puddle in the hole. LEL levels of 30 and CH4 IR levels of .75 were both detected in the hole with a gas meter. The fluid in the hole was field tested using a low range and a high range quan-tab. The low range registered 468 ppm Cl- and the high range registered 471 ppm Cl- content. There were two joint of 2 tubing on site as well as a small pit roughly 10 across. A 2 black plastic line came from well 2937 the Lackey #2 and was connected to the sales line at this well. This line was laid across the ground through the landowners backyard and field. Fluids could be heard moving through this line. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank each well-head shall be identified as to owner, permit number where available, well number and lease name. Any contaminated soils shall be removed and disposed of properly, topsoil shall be replaced and the affected area shall be seeded and mulched in order to establish vegetative cover. Leaking line shall be repaired or replaced. Any production lines laid in an area expected to be under cultivation must be buried at least 24 inches.

-1928831085 2013-05-23 Follow up to -1174478103. There is still a small are around the well head that has contaminated soils. The 2 sales line has now developed a different, smaller leak near the well head. I contacted the pumper for the company.
2110576596 2013-05-02 Follow up to -1174478103. Area around well head seeded, mulched, and limed. The main sales line is no longer leaking. The well is equipped with an identification. Tank battery equipped with two 100 bbl steel tanks, a separator, and a meter box.
