2399485 Primary Facility 2015-08-06 Follow-up Inspection No Violations Noted On 8/6/15, I performed a follow up inspection on the Alpha 1 site meeting with EQT representative Corey Giles to gather further information about a production fluid spill that occurred and was reported on 7/27/15:
2448362 Primary Facility 2016-02-04 Administrative/File Review No Violations Noted An administrative compliance evaluation inspection in response to the 8/6/15 Alpha 1 NOV indicates that as of 2/4/16 the situation is ready to be forwarded to the Environmental Cleanup Board. The dispensation of the site is as follows:

• On 7/27/15 EQT’s Corey Giles reported that “the containment for the sand tank accumulated 5,300 gallons of produced fluid. The containment overflowed releasing an additional 3-4 barrels to the pad surface.” • Administrative review of the initial clean up report and NOV response left some questions and issues unresolved. • An update to the initial response reviewed by DEP Kevin Maskol resolves all issues preventing the situation being taken over by ECB.

2532581 Primary Facility 2016-10-14 Routine/Complete Inspection No Violations Noted On 10/14/16 at approximately 1130 hours I conducted a routine complete inspection of the Alpha 1 pad. The Act 9 sign was in place and all required documents were present at the site. A drawing found in the mailbox (see photographs) roughly matched the site as constructed. According to Department records, this plan was not approved, nor were the changes made to the site.

Multiple violations of 102.5(m)(4) 102.5(c) were observed at the time of inspection and are listed below:

-The road to the south of the well pad was not constructed in the approved location -The gravel area to the south of the well pad was not constructed as approved and an increase in impervious area has resulted. -The location approved as an impoundment was constructed as an AST pad and is a significant increase in impervious area.

The area between the well pad, gravel pad to the south, and the location that was approved as an impoundment contained a large volume of water. The constructed features in this area are effectively working as
