0017035393 2016-01-11 Urban Drill / Deepen / Reopen No Violations I was on location @ 0730 hrs. to meet with Roy Poulson, Tom Wood and Discovery Oil and Gas personnel ( Dave Matak, Mike Ellenis ) and discuss plans to stop the water flow. After a meeting between Poulson, Wood and Discovery a decision was made to plug and abandon the wellbore and not proceed to scheduled total depth. Initial plan details were 1.) Mix weighted mud system ( 11.2 ppg ), 2.) obtain static conditions downhole, 3.) Remove drill string while keeping hole static, 4.) set wireline bridge plug to stop flow, 5.) set 500 cement plugs back to surface with drill pipe. The wellbore remained shut in under the Blowout Preventor ( BOP) while supplies and services were being assembled. No violations identified during this inspection.
0267506309 2016-01-14 Urban Drill / Deepen / Reopen No Violations I was on the Luck Unit # 3 location to observe the circulation of a balanced mud system to allow the removal of the drill string ( 6.25 drill collars and 3.5 drill pipe ) and proceed in the plugging and abandonment of the well bore. The decision to remove the drill string was made after the flow out of the choke lines had stopped. The weight of the mud returns was scaled at 11.5 ppg after going down hole with 12.3 ppg. At 1630 hrs. the Blowout Preventor (BOP) was opened and the fluid level was static. Poulson Drilling Corp. ( PDC ) began to slowly ( to prevent swabbing ) remove the drill string in ten (10) joint increments. A fluid volume ( 18.75 barrels ) equal to pipe being removed was loaded down the annulus. This procedure continued until the rig had completed the removal of the drill string at 2025 hrs. Appalachian Well Surveys (AWS) moved in and rigged up to set a wireline bridge plug at approximately 2490 to 2492KB. AWS strung their wireline through a lubricator which was set in the BOP and closed upon. This afforded a control measure in case the flow would start again. AWS slowly lowered the bridge plug to 2490KB and set the tool at that depth at 2138 hrs. PDC lowered 3.5 drill pipe ( slick ) into the well bore and tagged the bridge plug at 2490 KB. Petroset Cementing Services (PCS ) connected to the drill pipe and mixed a cement slurry ( 180 sacks of Class A, 38 barrels ) and displaced the cement slurry with 15 barrels of fresh water. The plugged interval was calculated to cover approximately 500 in a 8.75 well bore. PDC removed and cleared enough drill pipe to remain above the cement plug top. A wait on cement (woc) time of 12 hours was given to achieve adequate compressive strength to be reached due to low bottom hole temperature and mix water. Plugging operations were to continue after the wait on cement time had expired. No violations.
0318683554 2016-01-13 Urban Drill / Deepen / Reopen No Violations I was on the Luck Unit # 3 location to observe the the stabilization of the brine flow coming from approximately 2649 KB ( Bass Island Dolomite ). Poulson Drilling Corp. ( PDC) and Halliburton (HES) reconstructed the mud system to achieve a minimum weight of 12.5 ppg. It was observed that the flow ( through the chokes ) was diminishing once the mud weight reached 11.2 ppg. The day was spent rebuilding the mud system in .2 ppg increments as to minimize any risk of exceeding the Frac Gradient for the open hole section of the wellbore. PDC and HES were scheduled to continue to condition the mud system until the mud parameters were sufficent to balance the the brine flow out the choke lines. No violations identified during this inspection.
0319106220 2016-01-12 Urban Drill / Deepen / Reopen No Violations Halliburton Services was on location to build a mud system capable to stabilized the brine flow encountered in the wellbore at 2649 KB. Mud parameters were: wgt~ 10.7 ppg, Cl~ 90k, Ca~ 14k, viscosity~ 80, hardness~16 and yield point~ 15. Petroset Cementing Services (PCS) was on location to circulate the mud system. A pre-job meeting was held @ 1520 hrs. and displacement down the drill string started at 1530 hrs. The mud system was displaced at < one (1) bpm and 250 psi. After 20 barrels of displacement the pressure increased to 450 psi. with returns out the choke line flowed at 1.3 bpm. At 1709 hrs. the choke pressure decreased to 125 psi. Kill mud was seen at surface at 1730 hrs. after displacing 188 barrels. Return mud weight was measured at 9.7 ppg. The wellbore was still flowing out the choke line. It was determined that more rate was required to place the weighted mud across the flow zone before it became diluted. The circulated system was reconfigured to utilize the rigs mud pump. A total of 218 barrels of weighted mud was pumped and the mud weight could not be stabilized due to dilution downhole. Wellbore was shut in while rebuilding of the mud system.
0649807052 2016-11-02 Final Restoration No Violations The Luck Unit # 1 was plugged ( 1/15/2016 ) due excessive water flow from the Salina Formation. The well was plugged with Poulson Drilling over the hole. No pipelines or tank battery was constructed. The road were existing previously and were just fortified with additional material. The area of disturbance has been reseeded and is growing and stable. There were not any erosional features observed during my inspection. The Luck Unit # 1 met all criteria for final restoration.
0656048288 2016-01-15 Urban Drill / Deepen / Reopen No Violations I was on location to witness the plugging and abandonment of the Luck Unit # 1. The hole was lost due excessive brine flow from 2649 KB. and was being plugged from 2490 KB to surface with cement. After setting for 12 hours the 9.625 casing was still full and static. Poulson Drilling Corp.(PDC) lowered drill pipe down the hole to tag the top of the previous cement plug ( @ 1990 ). PDC found the top of the initial cement plug at 2087KB. The plug was 97 lower than the calculated fill volume. Four (4) subsequent cement plugs were set across 500 intervals by Petroset Cementing Services (PCS). Cement weights were verified by scale and ranged from 14.9 ppg to 15.4 ppg ( averaging 15.2 ppg ). After circulating the fourth (4th) plug PDC removed all the drill pipe and began to remove the BOP. After the BOP was removed an eight (8) foot pup joint was attached to the 9.625 casing in order to bring the casing above grade. After two (2) hours PDC lowered the drill pipe into the hole and tagged the top of the cement plug at 577 KB ( vs 575 KB calculated top ). Petroset Cementing Services mixed and circulated a cement slurry back to surface and out into the pits. PDC removed all ( 13 ) but three (3) joints of drill pipe to which PCS connect and circulated cement back to surface. After removing the last three (3) joints I could see the cement slurry down approximately 30 . Petroset Cementing Services topped the 9.625 casing to approximately three (3) feet above grade at 2120 hrs. The 9.625 casing was full and static when a 9.625 X 8.625 collar, 8.625 X 2 bell swage with 2 ball valve and tapped bull plug. A pressure gauge was attached to the bull plug to monitor well conditions. The casing will be capped with a welded plate and identified with the required information. No violations identified during this inspection.
0675894727 2016-01-07 Urban Drill / Deepen / Reopen No Violations I was on the Luck Unit #1 location to witness the testing of the BOP by Poulson Drilling. After being connected to the 9.625 casing the BOP was closed ( nitrogen bottles ) on 6.25 drill collars. Starting at 0152 hrs and continuing to 0207 hrs. the annular space below the BOP was pressured up to 400 psi and held static without any pressure decline. The test was passed. No violations.
1205652022 2016-01-10 Urban Drill / Deepen / Reopen No Violations I was contacted by Tom Wood ( Discovery Oil and Gas Consultant ) informing me that a large water flow had been encountered while drilling the Luck Unit # 3. Poulson Drilling Corp. ( PDC ) encountered a water flow while drilling at a depth of 2581 KB. This flow was described a as trickle . PDC continued drilling ahead and encountered an increase in the flow at a depth of 2620 KB and noticed a pressure increase on the choke lines up to 250 psi. A third increase in the water flow was noticed at a depth of 2649 KB. At this depth the flow was estimated initially at 500 barrels per hour and was a full seven (7) inch flow. The pits were filled within one (1) hour. After the chole manifold was blown off the end of the flow line, the Blowout Preventor ( BOP ) was closed on the 3.5 drill pipe. A hard shut in was maintained in order to repair choke manifold. Shut in pressure was 200 psi and slowly increasing. I scale the water, from the flow, and found it to weigh 9.1 ppg. Subsequent weight was found to be 8.7 ppg. A QuanTab analysis of the fluid indicated a Chlorides count of 6,178 ppm. Fluid temperature was 82.4 F. After the flow was encountered five (5) additional 500 barrel frac tanks were ordered to contain the flow during a kill attempt. Well was shut in overnight waiting on orders from the Operator.
1221043167 2016-01-19 Plug / Plug Back No Violations I was on the Luck Unit # 3 location to check the effectiveness of the preceding plugging of the lost hole. The above ground monument consists of approximately three (3) feet of 9.625 casing with a 9.625 X 8.625 collar, 8.625 X 2 swage with 2 ball valve and pressure gauge. I requested that Poulson Drilling Corp. personnel open the 2 valve in order to determine if any pressure had built up while shut in for five (5) days. After opening, I did not see any indication of pressure on the gauge. Poulson Drilling Corp. was rigging down the rig and moving off location. I will contact Discovery Oil and Gas to determined when a permanent identifing well cap will be affixed.
1232678424 2016-01-06 Urban Drill / Deepen / Reopen No Violations I was on the Luck Unit # 1 location to observe the placement and cementing of the 9.625 surface casing. Poulson Drilling ( PDC ) drilled a 12.25 bore hole from 280 to 491 KB on fresh water mud. Os Power Tongs Service ran 11 joints of 9.625, 26 lb/ft casing to 478 GL. Petroset Cementing Services mixed a cement slurry consisting of 175 sacks ( 37 barrels slurry ) Class A blended with 3% CaCl and .5 lb/sack celloflake. Displacement was achieved utilizing a five wiper plug and 36 barrels of fresh water. Casing hardware included 9.625 guide shoe and 4 - 9.625 X 12.25 bow spring centralizers. Upon completion of the displacement volume I did not see any cement slurry returned to the cellar. I requested that the 9.625 casing be held static for eight (8) hours and a Cement Bond Log be ran to evaluate the cement job. At 1900 hrs. Tri State Well Services ran a segmented bond evaluation log. The log found the cement top to be at 40 and the rest of the cement sheath to be of adequate quality. I gave permission to contiue to drill the Luck Unit # 1 to total depth with the provision that the annular space between the 9.625 and 13.375 be cement from 40 to surface. No violations.
1703758893 2016-01-04 Urban Drill / Deepen / Reopen No Violations The Luck Unit #1 was spudded on 1/5/2016 by Poulson Drilling ( PDC) @ 1315 hrs. PDC drilled a 17.5 bore hole to 280 KB on fresh water mud. A fluid loss zone was encountered at 130 KB. Os Power Tongs ran 6 joints of 13.375, 36 lb/ft casing to 253. Petroset Cementing Services mixed a cement slurry consisting of 350 sacks ( 73.5 barrels ) of Class A cement blended with 3% CaCl and .5 lb/sack celloflake. The cement slurry was circulated to surface with seven (7) barrels of cement slurry being observed in the cellar. After a waiting period of eight (8) hours PDC tagged the cement at 233. No violations
2050102336 2015-12-29 Urban Drill / Deepen / Reopen No Violations I inspected the Luck Unit #1 location construction prior to Poulson Drilling moving on and rigging up. The location was leveled and benched across an area of 150 X 200. I found the silt fencing on the East side of location to be inadequate. Once the site was level it was determined that the Southern and Western perimeters of the location will require erosional controls. I spoke with Dave Matak ( Discovery Oil and Gas ) about the modifications and additions. Mr. Matak agreed to place additional erosional controls once the pits are dug and the drilling rig is rigged up as to not disturb the erosional controls. The silt fencing on the East side location shall be repositioned upon the benched area after the rig move is complete. No violations identified during this inspection.
