| On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing.  At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing, and 13.375-inch diameter intermediate casing.  Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid storage tanks, and a combustor that was shared with the other producing wells on the pad.  There was secondary containment around the production fluid tanks, and the Gas Processing Units.  The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location.  There was a midstream valve site on the west side of the pad.  As well as a water impoundment to the south, right side of the lease road as you enter the pad.    Identification tags were present, and production was up to date.

Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0102875123 2014-05-29 Drill / Deepen / Reopen No Violations My inspection finds the drilling rig, Patterson 332, is currently drilling at a depth of 17,276. The anticipated the total depth is 17,709. The rig will be running 5.5 twenty pound casing.
0199057673 2017-03-17 Production Wells No Violations The pad is named Wilson and it consists of 8 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. Antero Resources was on location preforming maintenance on a gas production unit.
0491805055 2015-12-28 Production Wells No Violations My inspection today finds the well is in production. The well is still being produced into temporary tanks while work on the North slope of the pad is on going.
0544902763 2016-06-17 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of eight 400 barrel tanks set inside of a plastic lined steel containment system. On the opposite side of the pad there are eight production units, also set in containment.
1092162125 2014-02-25 Casing String Witnessed No Violations My inspection finds Express Drilling ran and cemented 100 of 26 conductor. I witnessed returns to surface.
1139334577 2015-11-13 Production Wells No Violations My inspection today finds the well is capable of production. A small slip has formed on the Northeast side of the location. The engineering group is monitoring the slip. A geotech drilling rig recently drilled a test hole on the well pad to test for stability.
1396961632 2014-04-07 Production Wells No Violations My inspection today finds the drilling rig is at a depth of 130 and currently drilling 24 hole in the surface section of the well.
1436410713 2019-01-31 Production Wells No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing.
1517848467 2018-02-02 Production Wells No Violations Status check following a nearby pipeline mechanical issue that is not owned by Antero. This pad is named Wilson and it consists of 8 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. The pipeline tie in was located on the West side of the pad with no issues present. The identification was located at the wellhead and the production records were current.
1521463265 2018-03-13 Production Wells No Violations The purpose of my inspection was a status check. The pad is named Wilson and it consists of 8 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. The production equipment consisted of eight gas production units, eight 400 barrel tanks, two vapor recovery towers, and two combustors. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.
1577405213 2015-01-13 Preliminary Restoration No Violations My inspection today finds the well site meets preliminary restoration requirements. The tank battery consists of eight 500 barrel tanks set in a liquid tight containment.
1604203400 2019-01-31 Production Wells No Violations On site inspection along with ODNR Inspector Ryan Armstrong. On this day we were conducting field training and visited the well pad. The pad consists of 8 producing wells. The tank battery sits on the left side of the pad as you enter it and consists of (8) 400 barrel tanks. Identification is present on each well head. See Ryans reports for more information.
1710601121 2017-10-24 Production Wells No Violations The purpose of my inspection was a status check on a production well. The pad is named Wilson and it consists of 8 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. A gauge present on the production casing read 330 pounds per square inch and the gauge on the production tubing read 190 pounds per square inch. Annular pressure monitoring was present. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.
1758576358 2014-04-21 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Dallas Morris 24, is beginning to run forty pound 9.625 casing. Halliburton will be providing cementing services.
1897145475 2014-04-08 Casing String Witnessed No Violations My inspection finds the rig, Dallas Morris 24, ran 417 of 18.625 casing. Allied cementing circulated 175 bbl of cement into place to seal the casing string. I verified the cement weight at 15.6 pounds per gallon. I witnessed 57 bbl of cement returns to surface.
