| On the day of my inspection I met with Vernon King and Jeff Redman to pressure test the surface casing cement job. They pressured the casing up to 340 psi and shut it in. The pressure fell 20 psi over a period of 30 minutes. I approved the test as passed. The surface casing cement had fallen approximately 30 feet. Vernon and Jeff topped the annulus off with nine (9) sixty (60) pound bags of sack cement.

| On the day of my inspection I found that the well was equipped with an electric powered pumping unit. The motor, drive belts and electric were intact. The flow lines were connected at the wellhead and the tubing valve was open. The permit number was painted on the pumping unit. There was a slow trickle of gas venting out of a gate valve on the casing head, and there was a small spray of oil in the weeds extending out from a bleeder valve on the tubing. The production equipment is shared with permits 7229 and 7244. It consisted of two 100 barrel steel tanks in an earthen dike. Correct, legible identification was painted on one of the tanks. There were bull plugs in the tank valves, and there were no leaks or spills in the dike. I called Jeff Redman and left a voice message to contact me in regards to getting a pop-off valve installed for venting casing gas and using a receptacle to catch the oil spray when they open the tubing valve.

| I contacted Vernon King in regards to the drilling status of the well. He claimed that he hit a crevice below the 8.625 casing that was caving in while drilling. He notified me that he intended to run 7 casing to approximately 60 and then pull the 8.625 casing. The 7 would then serve as the conductor. Rednet will run 4.5 surface casing in order to meet the annular spacing requirements.

| On the day of my inspection I found that a group of trees on the edge of the forest, directly behind the location of the drilling pit were showing signs of distress. The leaves were completely brown on some trees, and the edges of the leaves were brown on others. The rest of the trees along the forest edge away from the pit location were completely green. The drilling pit has been closed with just a mound of loose soil remaining. It was reported to me that the pit was closed on 8/2/16. There were some deer tracks in the moist portion of the reclaimed area, but no signs of heavy deer traffic. I walked the forested area behind the former pit location and I did not find any visual evidence that would suggest the pit water was drained over the hill. The only vegetation that appeared to be affected was the group of trees along the edge of the forest. I collected some soil and mixed it with drinking water to conduct a field test for the presence of chlorides. I tested the water with both low and high range Quantab test strips. Both strips were consistent with approximately 480 parts per million. I took photos of the trees in relation to the pit location and the unaffected trees. I contacted DOGRM Hydrogeologist Jake Glascock about soil sampling. Jake suggested further investigation before conducting laboratory soil sampling. I called Jeff Redman. He did not answer and there was no option to leave a voice message. I will continue to follow up.

| On the day of my inspection I found that all materials and debris had been removed from the well site. The soil had been graded, seeded and covered in straw. A diversion berm had been cut into the slope to direct rain water away from the well site.  The area was 80% covered in new grass growth. I found no signs of erosion, and I found no signs of tree damage from the pit closure incident last summer. The permit number was posted on the pumping unit and production tank. This site passes preliminary restoration.

| On the day of my inspection I found that Vernon King was drilling at a depth of 350 with 6.5 bit. The permit was posted in the shante. The fluid level in the drilling pit was low enough that there was no danger of breeching the pit walls. There were no sags or visible tears in the pit liner. The ground was snow covered, but I found no signs of leaks or spills around the drilling rig.

| On the day of my inspection I found that the drilling rig, and all related materials and debris had been removed from the well site. The site had been 90% graded, with a mound of loose soil where the pit was located. I spoke with Jeff Redman. He claimed that they would be back out to finish grading the site. I spoke with Vernon King. He claimed that he would be out this weekend to seed the area. The well is located in an agricultural crop field. I told Vernon that there was no need to seed the entire graded area since it will be plowed and planted. I suggested that he seed the immediate area around the pumping unit where no crops will be planted. There were no signs of erosion at the well site.

| I made another attempt to contact Jeff Redman. He did not answer and his voice mailbox was full. I called Vernon King. He claimed that they did have the pit drained prior to closing. Vernon acknowledged the stressed trees, and suggested that there may have been a small hole in the pit liner. I personally visited the drill site on a weekly basis throughout the drilling process. I did not notice the leaves turning brown during those visits. I also kept a close watch on the fluid level in the pit. I did not observe any evidence that fluid was breeching the top of the pit, and I did not observe a noticeable drop in the fluid level at any time throughout the drilling process. Vernon Agreed to meet with me at the site today.

| On the day of my inspection I found that the well site had not been final graded or seeded for erosion control. There was still a mound of soil where the pit was located. Most of the site was still bare and there were small erosion ditches forming where the rain water was running off into the woods. There was an empty 275 gallon plastic cube container next to the wellhead. There was some oil staining in the soil around the wellhead, but nothing fresh. This well shares a common tank battery with permits 7229 and 7244. There was no identification at the wellhead and this well had not been added to the identification at the tank battery. I called Jeff Redman to go over the issues. He said that they would be out within the next two weeks to take care of the problems.

| On the day of my inspection I found that Vernon King was drilling into the Berea sandstone at a depth of 942. There was an oil and gas show. Vernon bailed approximately 4.5 barrels of oil into a 375 gallon steel tank. He equipped the 4.5 casing with a swage and ball valve. Vernon had the valve open and there was a slow steady trickle of gas flow. Jeff Redman arrived with fittings to run a line from the casing valve to the pit should the well decide to flow oil. Vernon said that he wanted to drill 20 feet deeper in to the formation to allow for sand deposits in the bottom of the hole.

| On the day of my inspection I noticed fluid in the dike when driving by on TR 205. I stopped to investigate and found oil staining on the gravel below the load valve on one of the 100 barrel tanks. The fluid was cloudy and covered in an oily skim. I tested the water for chlorides with a low range Quantab and it showed none. I did not see any evidence that the fluid had escaped containment. There was no staining on the outside of the dike or in the lease road next to the dike. I called Vernon King to report the issue and to request that they get it cleaned up. He said that he would talk to Jeff Redman and they would get it taken care of.

| On the day of my inspection I found that the soil was moist from recent precipitation. I collected another soil sample from the surface, in the dampest area, next to the tree line. I mixed the soil with purified drinking water in order to test it for chlorides with a Quantab test strip. The test showed approximately 400 parts per million (ppm). There was no change from the field tests conducted on 8/10/16. I found that the affected vegetation included four (4) sugar maples, one (1) American elm, and one (1) multiflora rose bush. Vernon King met me at the site. He did not dispute the fact that the trees were exhibiting signs of exposure to oilfield brine water. Vernon did maintain that the pit was drained prior to being pushed in. He admitted that there may have been less than 100 gallons of fluid left in the pit that they were unable to pump out. He said that the fluid was pumped to the tank battery with a portable gasoline water pump. Vernon said that he had run tickets from having the water hauled from the tank battery. He claimed that the run tickets would show that there was an increase in volume being hauled from the tanks. I told Vernon that we would need to do some more investigation before determining a course of action.

| On the day of my inspection I met with Emergency Operations Supervisor Trisha Cartmell to examine the stressed vegetation at the well site. We found that 90% of the graded drill site and closed pit location was covered in grasses and weeds. The affected trees did not show any worsening in condition. There were no new trees or understory vegetation showing signs of stress. Trish and I both agreed to take no further action at this time. I will continue to monitor the site, paying close attention to the physical state of the affected trees in the Spring of 2017, before requiring Rednet Exploration to remediate the soil.

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0052011640 2016-03-24 Drill / Deepen / Reopen No Violations On the day of my inspection I met with Vernon King and Jeff Redman to pressure test the surface casing cement job.
0102915899 2018-01-25 Production Wells No Violations On the day of my inspection I found that the well was equipped with an electric powered pumping unit.
0119013776 2015-12-07 Drill / Deepen / Reopen No Violations I contacted Vernon King in regards to the drilling status of the well.
0153302122 2018-08-10 Production Wells No Violations On the day of my inspection I found that a relief valve had been installed on the casing nipple to vent casing gas. The electric motor, drive belts and power supply were intact. The flow lines were connected and the valves were open. I found no leaks around the wellhead. There were no leaks or spills around the tanks. The permit number was written on the pumping unit and correct, legible identification was painted on the tanks.
0169776481 2016-05-19 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was not drilling. I called Vernon and he said that he was at a depth of 962. There were no sags in the pit liner.
0204140418 2016-06-10 Drill / Deepen / Reopen No Violations On the day of my inspection Vernon King was cleaning out the hole. The well is drilled to a total depth of 962. Vernon will continue to clean the sand out of the hole until he feels it is ready for tubing.
0209066028 2016-08-10 Preliminary Restoration No Violations On the day of my inspection I found that a group of trees on the edge of the forest, directly behind the location of the drilling pit were showing signs of distress.
0221380676 2017-06-07 Preliminary Restoration No Violations On the day of my inspection I found that all materials and debris had been removed from the well site.
0289151882 2016-01-13 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was not drilling. The rig was still down for repairs. There were no sags in the pit liner and I found no leaks or spills.
0647504469 2015-12-22 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was not drilling. I called him and he said that he had to drive 5 more feet of 7 for a total of 66. He has not cemented the 7 yet, and he is still at a depth of 100. There were no sags in the pit liner and I found no leaks or spills.
0698473689 2016-02-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was not drilling. There were no sags in the pit liner and I found no leaks around the drill site. There were fresh tire tracks in the snow leading back to the well. I called Vernon King and left a message to contact me in order to report the drilling status and a current depth.
0738464960 2016-03-04 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was drilling at a depth of 350 with 6.5 bit. The permit was posted in the shante. The fluid level in the drilling pit was low enough that there was no danger of breeching the pit walls. There were no sags or visible tears in the pit liner. The ground was snow covered, but I found no signs of leaks or spills around the drilling rig.
0928412944 2016-08-05 Preliminary Restoration No Violations On the day of my inspection I found that the drilling rig, and all related materials and debris had been removed from the well site.
0931397398 2016-03-08 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was not drilling. The well bore was covered with a bucket and the tools were positioned over the bucket. The fluid level was low in the pit and there were no sags or visible tears. I found no leaks or spills around the drill site. I called Vernon King and he reported that he was at a depth of 375.
0948334194 2016-04-06 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was drilling at a depth of 580. There were no sags or visible tears in the pit liner and there were no leaks or spills around the drill site.
0950634459 2016-08-11 Preliminary Restoration No Violations I made another attempt to contact Jeff Redman.
0972428240 2016-05-24 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King and Jeff Redman were on location testing the well for oil production. They wanted to evaluate the wells capability prior to making the decision to shoot the well or let it produce naturally. Vernon reported being at his target depth of 962. He said that he has bailed approximately 1800 gallons of oil from the well in two weeks.

1144057537 2017-04-24 Preliminary Restoration No Violations On the day of my inspection I found that the well site had not been final graded or seeded for erosion control.
1191453299 2015-12-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was not drilling. There were no sags or visible tears in the pit liner and I found no leaks or spills around the drill site.

1232602187 2016-06-17 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was finishing up with the clean-out of the well bore. The well is making oil and it is ready to be equipped for production.
1300654552 2016-05-02 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Vernon King was drilling into the Berea sandstone at a depth of 942.
1325111104 2017-07-18 Production Wells No Violations On the day of my inspection I noticed fluid in the dike when driving by on TR 205.
1422360280 2019-03-14 Production Wells No Violations On the day of my inspection I found that the motor, drive belts and electric line were intact at the pumping unit. Vernon King was on site pumping the well. The drilling rig was still on location, and the pit was still open. I found no leaks or spills around the drill site.
1471016459 2016-08-11 Preliminary Restoration No Violations On the day of my inspection I found that the soil was moist from recent precipitation.
1474298076 2016-09-19 Preliminary Restoration No Violations On the day of my inspection I found that two of the affected sugar maple trees and the multiflora rose appeared to be recovering slightly from brine stress. There was no observed change in the condition of the ash tree, and one sugar maple. There were no new trees observed to be showing signs of stress. The reclaimed drill site was approximately 80% covered in grass and weeds.
1475253073 2016-09-29 Preliminary Restoration No Violations On the day of my inspection I met with Emergency Operations Supervisor Trisha Cartmell to examine the stressed vegetation at the well site.
1601483599 2016-03-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Vernon King Drilling had drilled a 6.625 inch diameter hole to the depth of 440 feet and set 440 feet of 4.5 inch 10.5#/ft. used tested to 4400psi and end drifted to 3.9. Water was tested revealing a pH of 6 and 88ppm Chlorides. Formation Cementing was onsite gained circulation with 20bbls of gel and 60lbs of cotton seed hulls. Formation cemented casing with 73 sacks of Class A Cement weighing 15.6ppg and a yield of 1.18cuft/sk. with 70lbs of flake added. Cement was displaced to 420 feet with 6.7bbls of fresh water with good cement returns of 4bbls to surface. Cement dropped to 12 feet after displacement and operator was instructed to top off to surface.
