+ | | I received a phone call from Rick Liddle of Ohio Valley Energy in order to provide notification of an injury accident that occurred on Wildcat Drilling Rig #1 at approximately 4:00 pm on Saturday, March 24, 2012. It was reported that a cable broke while operating power tongs and resulted in a rig hands leg being severed approximately 6 inches below the knee. I did not witness the injury and no further information was provided.

+ | | No Violations | ||
+ | | I was on location to pass preliminary restoration for this well. The lease road is graveled, in good condition and has a gate with I.D. signage. It leads up to a fenced in pump jack well in good mechanical condition with proper gauges and locks. The tank battery is surrounded by a dike, fencing and has I.D. signage and contains a two 100bbl production tanks, poly brine tank and a mechanical separator. At this time preliminary restoration has been passed.

+ | | I was on location to follow up on an oil spill that occurred the day before. Currently all the unfrozen oil has been cleaned up. The gates of the wellhead have been removed so that they can be cleaned at the shop. Adsorbent is around the spill and the well to catch the thawing oil. The site is being checked twice a day and the crews will be working through the weekend to ensure no oil leaves location. There are just a few spots of frozen oil left that cannot be reached until the thick ice and snow softens up.

+ | | I was contacted by Ohio Valley Energy informing me that there has been an oil spill at this location. When I arrived on location there was a 10 by 15 by 3 oil spill around the pump jack and the fencing. I estimated the spill to be 5 barrels of crude oil. It was believed that the spill was caused by the below freezing oil temperatures. The oil separator was pulled dry of gas from the house gas hookups. This tripped the safety shutoff on the production tanks. The safety shutoff did not engage on the pump jack which caused the pump jack to cycle and blow out the bushings on the polishing rods. This caused the well to pump crude oil onto the ground. The spill was sucked up by a vacuum truck. The rest of the oil was shoveled up down to the ice. It is believed some oil has gone under the ice in the area. Adsorbent has been placed around the location and a skid-steer was brought on location to scrape up and clean up the oil when the ice thaws. Ohio Valley will be checking the location twice a day to clean up oil as it frees from the ice and the location has been contained. No oil has left the location and is well contained at this time. I will follow up on the spill to confirm it has not spread and has been thoroughly cleaned up.

0123763833 | 2012-03-27 | Urban Drill / Deepen / Reopen | No Violations | I received a phone call from Rick Liddle of Ohio Valley Energy in order to provide notification of an injury accident that occurred on Wildcat Drilling Rig #1 at approximately 4:00 pm on Saturday, March 24, 2012. It was reported that a cable broke while operating power tongs and resulted in a rig hands leg being severed approximately 6 inches below the knee. I did not witness the injury and no further information was provided. |
0197109476 | 2012-03-30 | Urban Drill / Deepen / Reopen | No Violations | I was on location to check on the status of the pit closures. John Rose Construction was at the well site closing the pits. The pits will be solidified and hauled away. |
0223559820 | 2012-03-16 | Urban Drill / Deepen / Reopen | No Violations | I was on location to witness the blow out preventer test. The pressure was held at 600 psi for 15 minutes with no bleed-off. The blow out preventer passed the pressure test. Wildcat Drilling Rig #1 was cleared to continue drilling. |
0296929690 | 2012-03-20 | Urban Drill / Deepen / Reopen | No Violations | Wildcat Drilling Rig #1 was drilling on fluid with a 7-7/8 bit and they were at a depth of 4170. The correct permit was displayed at the site entrance and in the doghouse. Silt fence was in place to the south. The entire location was fenced with temporary orange fencing and the pits were fenced with chain link. There were no tears or sags in the pit liners and no leaks or spills around the drilling rig. No violations at this time. |
0810852137 | 2014-08-19 | Preliminary Restoration | No Violations | I was on location to pass preliminary restoration for this well. |

1010283950 | 2014-01-09 | UrbanProduction Wells | No Violations | I was on location to follow up on an oil spill that occurred the day before. |
