1965150 2011-04-06 No Violations Noted US Energy rig shut down at time of inspection. Crew in well control training.

13 3/8" x 7" annulus - cement not visible at surface. Pit lined and freeboard maintained.

1965157 2011-04-12 No Violations Noted At time of inspection, US Energy tripping out of hole after drilling to 3020' TD.

Information per cement job log: 1011' of 20 lb/ft 0.250" wall 7" casing set in 8 7/8" hole and cemented with 70 sacks of "A" containing 3% cacl2 & 1/4# flake (yield=1.54 cu ft/sk); tailed by 75 sacks of "A" containing 3% cacl2 & 1/4# flake (yield=1.18 cu ft/sk). Cement returns to surface realized and bumped plug at 16:00 04-04-11. Drilling rep Joe Miller stated that cement top in 13 3/8" x 7" annulus at approximately 3'. Pit lined and freeboard maintained.
