2559531 Primary Facility 2017-02-06 Routine/Complete Inspection No Violations Noted Arrived @ 10:40. Weather: Sunny, temperatures above freezing. At the time of my inspection Ziegenfuss Drilling was on site. Spoke with Lenny Long (Ziegenfuss) who was spudding conductor on the 4H and preparing to move to the 3H. Had replaced 20” conductor on the 1H with 24”, and would follow with the 2H. There were no violations at the time of my inspection. Left location @ 10:40.
2563560 Primary Facility 2017-02-15 Routine/Complete Inspection No Violations Noted Routine Inspection; 2/15/2017.

I entered the site off Sugar Camp Road at approximately 10:00. At the time of the inspection, conductor was set on four of the five wells onsite. The Ziegenfuss rig was packed-up but still onsite. A vac-truck was onsite cleaning out the single Frac tank. One diesel tank was onsite in containment and two roll-off containers onsite were holding cuttings from the conductor. The two center wells were labeled the Winter 1 (081-21474) and Winter 2 (081-21501), while the two additional wells were not labeled. Please label the remaining two wells as soon as possible (I understand the conductor were just set).

The well pad is well vegetated except for the recent construction of PCSM (Post Construction Stormwater Management) features on the eastern (lower) part of the site. On 2/1/2017, the Department approved a permit modification to add additional gravel pad surface to the well site. This construction of this additional impervious area has not been initiated. No Violations

2605243 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted Routine inspection; 6/15/2017.

I entered the site at 14:15 and talked to the company man (Carey Carr). They were in the process of tripping out of the Winter 1 (081-21474) to replace the BHA mud motor. Planning on finishing this third casing string before reaming run with the same diameter. Containment is in good condition and housekeeping looks good. Please be mindful of the chemical storage area. Chemicals are stored on containment as required but i recommend not storing chemicals so close to the edge when possible (see photos). The site is seeded and stabilized and no E&S issues were noted.

2635730 Primary Facility 2017-09-06 Routine/Complete Inspection No Violations Noted I entered the site with Clark Smith (DEP) and we talked to Greg Malekos (Inflection) before the inspection. At the time of the inspection, Inflection was drilling the Winter 2. Rig mats were very uneven on the north and east sides of the site apparently due to compaction and traffic on this recently expanded part if the pad. The areas under the rig and associated equipment did not show signs of settling. A sheen and some friction reducer lubricant was visible on water in containment near the compressors. I recommend vacuuming the water from containment. I also recommend vacuuming and careful management if water in containment when rigging-down.

I recommend evaluating the areas where the pad has settled after the rig moves offsite and before completions operations begin. No violations noted.

2650965 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted I met onsite with Jerry Roberts (company man) and then walked the site. At the time of the inspection, a Coiled Tubing rig was onsite rigging-up on the Winter 2 (081-21501). One tear in the containment liner was noted west of the well heads where rig mats were removed. The pad is uneven from the rig traffic and the condition of the containment remains unknown. Since the containment used for drilling will be reused for completions, I recommend individual containments around tank batteries (working tanks, etc.) and the blender. A modular fresh water tank is being installed on the auxiliary pad east of the well heads. Fresh water will be pumped to the site via temporary overland waterlines from the Bennett and Stunner-Smith sites. No violations noted.
2666733 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted I met with Jeff Whitehair on location. Cudd coil tubing rig currently drilling out plugs on the 1H well. Containment appeared to be clean and in excellent condition. A puddle (5-10 gallons) present near the company man trailer showed slightly elevated conductance (2.3 mS/cm); I recommended to Jeff that he have it vacced. I also noted a small (less than a gallon) puddle of a white liquid on containment behind the coil tubing truck, and I also recommended to Jeff that he have it cleaned. Once they are finished with the 1H, Inflection plans to move to the 4H and then the 2H to finish drilling plugs.
2676348 Primary Facility 2017-12-19 Routine/Complete Inspection No Violations Noted At the time of the inspection, the wells were drilled, completed, and awaiting flow-back. The secondary/auxiliary pad at the entrance was being prepared for an Aboveground Storage Tank (AST). The production facilities and flow lines were also being installed. Perimeter E & S controls were installed downslope of earthmoving activities. Overall the site was very busy. No violations noted.
2689803 Primary Facility 2018-02-01 Routine/Complete Inspection No Violations Noted Routine Inspection; 2/1/2018.

At the time of the inspection, three wells were in production and temporary sand separators were still connected. Sand separators were plumbed to frac tanks in containment on the southeast corner of the well pad. Containment was being installed around the production units and produced water tanks were in containment. Earth disturbance activities are ongoing to restore the site (except the AST). Earth disturbance activities were almost complete around the well pad area and seed and straw mulch was being applied. No violations noted.

2696276 Primary Facility 2018-02-21 Routine/Complete Inspection No Violations Noted Whether it's 50° and cloudy at time of inspection. Well as a spud conductor with the steel plate sitting over it. No violations noted at time of inspection.
2738457 Primary Facility 2018-06-07 Routine/Complete Inspection No Violations Noted Weather was 55° and sunny at time of inspection. There were no Inflection representatives on location at time of inspection. Well is a spud conductor with a steel plate welded over it. A well tag is in place with the proper API number on it. No violations noted at time of inspection.
