Well appears to be in production at this time. Site is restored and vegetation has achieved 70 % uniform coverage. The access road is in good condition ¿ BMPs in place. The Department notified the operator that the regulations state that ¿The well operator shall install the permit number issued by the Department in a legible, visible and permanent manner on the well upon completion.¿ The Department noted a small dent in the brine tank and the slight oil spill on the ground inside the secondary containment.

1901435 2010-07-26 No Violations Noted On July 26, 2010 I conducted an inspection of the above site in response to the explosion / fire incident that occurred on July, 23, 2010. The purpose of my visit was to assess any environmental impacts that may have occurred as a result of the explosion or subsequent fire fighting activities.

Also on site was Rich Thomas, DEP Solid Waste Specialist; Don Brucker, Allegheny County Fire Marshall; Robert Full, Allegheny County Chief of Emergency Services; Alvin Henderson, Assistant Chief of Emergency Services; and Robert Wilson, Indiana Township Chief of Police. (continued on page 2)

2021767 2011-11-18 No Violations Noted On November 18, 2011 I inspected Murry Heirs#3 well and Murry Heirs #6 well both in Indiana Township, Allegheny County and owned and operated by Huntley & Huntley of Monroeville.

Both wells were in production and their sites well-stabilized with vegetation. I found no problems on site except the #6 well did not have a tag identifying it.

On November 23, 2011, I talked to David Janco an independent consultant working for Huntley & Huntley, and he told me he would have the well tender tag the well.

2122742 2012-12-10 No Violations Noted John Ley WQS SWRO, was on site to complete a routine inspection. Drilling was completed and the well appeared to be equiped for production. Well was labeled. Brine tank located onsite appeared to be in sound condition. There was no observed accelerated erosion or sedimentation on the access road or well-site. The site had at least 70% vegetation on the hillsides.
