Moved onto this well on 2/10/15 and resumed drilling at 7406 ft. KB. Operator drilled to 7990 ft. on 2/11/16. The top of the Marcellus Formation was found at 7676 ft. The Onondaga Formation was tagged at 7807 ft. Currently TIH with 3 ½” fiberglass tubing to plug back to KOP.

Reentered this well on 2/19/16 and tagged cement plug at 6912 ft. KB. KOP was at 7188 ft. EOB on the curve was at 8237 ft. MD (7781 ft. TVD). TD was at 13,163 ft. MD (7684 ft. TVD) on 2/22/16. The 5 ½” casing was set at 13,153 ft. and cemented to surface with 10 bbls returned.

-operation inspection could not be completed for the site as the perimeter controls near the entrance including a large diversion ditch were under construction at the time of inspection. No violation. Pre-operation inspection not completed, site not ready for drilling.

2478242 Primary Facility 2015-12-02 Preoperation Inspection No Violations Noted On 12/2/15, I completed a pre-operation inspection for the Captain USA Well Site in response to an operator request and made the following observations:

-operations inspection to be completed.

    -Clay sediments observed in rock lined diversion channel may indicate insufficient lining thickness.  No violation, inspection and remediation recommended.

-accumulating immediately beyond perimeter controls on the northwest side of the area and may be causing accelerated erosion.

    -Samples taken from water pooled on site and from runoff taken for MBAS analysis due to significant observed foaming in the stream channel and in water pooling at site controls.

Snow cover in the area of concerned prevented effective inspection of the site.
2483822 Primary Facility 2016-03-08 Follow-up Inspection No Violations Noted On 3/8/16, I performed a follow up inspection with DEP engineer Emmanuel Nwaobilo for the Captain USA site in response to an ongoing issue with site E&S controls and made the following observations:

•Along the access road, where it crosses the last saddle before the pad, E&S controls have been heavily reinforced partially in response to department recommendations. •Inspection of the area below the reinforced controls found multiple streams and waterways not shown in the site plans or in the original streams and wetlands report for the site. Due to this the site is not in compliance with §102.4(b)(4)(ii),(iv), §102.8(b)(2),(3),(5), and (8), §102.4(b)(5)(v) and §102.5(m)(2)(ii). •Many of the stream channels in the area adjacent to the site appear to be undergoing increased erosion and sedimentation. This is a violation of §102.2(a), (b); §102.11(a)(1),(2) and 35 P.S. § 691.401 •The site was also noted to be in in the drilling/PCSM phase at the time of inspection and primary site construction was completed t

Each gas well Captain USA 11H (125-27816) and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was tagged with identification.
