2440004 Primary Facility 2016-01-11 Compliance Evaluation No Violations Noted At the time of inspection, production equipment was painted. Brine tank was at full capacity. A well tag was installed on the separator. Production reports and mechanical testing reports were submitted to PADEP. However, the production and MIA reports are still missing from the package submitted to the PADEP. At the time of the inspection, gas from the well is being supplied to the landowner’s resident dwelling.
2480745 Primary Facility 2016-04-28 Complaint Inspection No Violations Noted DEP representatives (Paul Wyrwas, Jason Brannon and Ed Ritzer) performed an inspection of the site. Complaint made relative to the well causing leakage of gas to surrounding area. A well tag was installed on the separator. DEP used the 62S meter (last calibrated on April 20th using three point system) and try to determine if any gas reading located around the well. DEP noted a small crack in the 8 inch reducer fitting. The crack in metal was approximately 1/2” in length. No readings from the meter were recorded surrounding the production equipment and site pad. A reportable reading of 40 percent gas was detected at the small crack. The well is currently running at 2 psi of pressure at the well head. DEP called Thomas Leeper and informed him of the crack in the well reducer fitting. DEP is also concern with the full capacity of the brine tank. Mr. Leeper agreed to repair the leak and remove the brine water from the tank
2517705 Primary Facility 2016-08-29 Routine/Complete Inspection In Compliance with Policy At time of inspection access road vegetated, no E&S issues observed. Well site vegetated. Department observed an area of standing water near the SW side of brine tank. Soil probe reading of 0.09 mS/cm appears to indicate that the fluid is not brine. Brine tank appears structurally sound and does not appear to be leaking. Brine tank does not appear to have been emptied since OGI inspection of 4-28-16 (inspection record #2480745).

Well label present and legible. Previous inspection by OGI sites a small crack in the well head casing. As of this inspection the crack has not been fixed and the well appears to be leaking. This inspection is PENDING. Violations may be issued after further investigation of OGI and WQS. UPDATE 10-3-16: After further investigation it appears that, at this time, there are no adverse affects observed at the well site due to the operation of this oil and gas well. However, Department recommends Operator continue to monitor and maintain brine tank to avoid any potential pollution events
