•Samples were taken for SAC 946 and bacterial analysis. •Water had no odor or discoloration.

2482090 Primary Facility 2016-02-17 Administrative/File Review No Violations Noted On 5/9/16, I performed an administrative review of sample results and site observation for a complaint related to the Charles Bello site and made the following observations:

•Water samples were found to have no elevated chemical parameters; coliform and e. coli bacteria presence were found indicating surface water influence. •Based on distance to nearest wells (~2600 feet away) as well as the above test results no oil & gas activity impact is indicated.

2490656 Primary Facility 2016-04-19 Incident- Response to Accident or Event No Violations Noted On 4/19/16, I performed an incident inspection at the Charles Bello site in response to an operator reported spill (CTS# 319113) that occurred on 4/18/16 and made the following observations:

•Per the operator’s initial report roughly 10bbl of reuse water was released to the pad surface, the spill was confined to the pad, and did not impact surface waters.

    -The loss of 10bbl of reuse water to the pad surface does constitute a violation of 58 Pa. C.S. § 3217(a) for Failure to control and dispose of brines produced from the drilling, alteration or operation of a well consistent with the Clean Streams Law and 25 Pa. Code § 78.54 for failure to control brine in a manner that prevents pollution of the waters of the Commonwealth.

•The initial report indicates that the spill was caused by overfilling a tank which caused material to be sprayed beyond the secondary containment. Weavertown Environmental Group was reported to have begun remediation on 4/18/16. •At the time of inspection initial material removal ha

2491497 Primary Facility 2016-06-06 Compliance Evaluation No Violations Noted On 6/6/16, I performed a compliance evaluation inspection for the Charles Bello site in response to a spill that occurred on 4/18/16 and made the following observations:

•At the time of inspection a large shallow pit partially filled with water was observed in the location of the spill. Water in this excavated area was found to have similar conductivity to standing water elsewhere on the pad not believed to be affected by the spill (between 675-700¿¿s). Living tadpoles were observed in the standing water throughout the site. •No construction or drilling activities were ongoing at the time of inspection. •Stabilizing vegetation immediately adjacent to the first half of the site access road was very sparse in many areas. No violation, remediation recommended. •An area of disturbed ground crossed the site’s access road and led to the pad near the production units. Contact with an operator representative indicated that the disturbance was attributed to a pipeline owned by Superior having been constructed. T

Well apparatus and brine tanks appear to be operational. Temporary erosion and sedimentation BMP super silt fence installed and functioning. Three PCSM basins appear to be constructed and functioning. Post-drilling well site restoration approved. No observation of accelerated erosion or sedimentation observed in relation the the well pad or access road. Disturbed area appears to have 70% uniform vegetative coverage.
