2630249 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted On 8/29/17 a RTNC inspection was completed at the Sandra Eakin well site in relation to notifications of additional wells being drilled. The following was observed:

>>The well site was being prepared for additional drilling at the time of inspection. No rig was on site at the time of inspection.

>>The site was in generally good condition with fresh gravel and other site maintenance being conducted at the time inspection.

2638384 Primary Facility 2017-09-21 Site Restoration No Violations Noted On 9/21/2017, WQS Brian MacQuarrie onsite to meet with Nick Reda- Range Resources. Falcon Drilling Rig #27 onsite. The tophole section of two of seven additionally permitted wells for this site have been completed. Currently five wells are in production. The site is well vegetated and sediment basin has been converted to wet retention pond.
2641075 Primary Facility 2017-09-28 Incident- Response to Accident or Event No Violations Noted On 9/19/2017 an INCDT inspection was conducted at the Sandra Eakin well site in response to a spill reported by the operator on 9/15/17. The following was observed:

>>A spill of 25 gallons of drilling water resulted from a hole in a transfer hose. The spill was reported to have been contained on the pad. This constitutes a violation of 25 Pa. Code 78.54 and 78.57(A) in that the operator failed to properly control and dispose of production fluids.

>>All spilled liquid and contaminated pad material was reported to have been gathered by a vac truck and removed for disposal. Testing for extent of spill was conducted using conductivity.

>>Inspection of the site found that the initial incident report appeared to be accurate and cleanup was completed as reported with no observed indication of residual contamination.

2649666 Primary Facility 2017-10-13 Drilling/Alteration No Violations Noted Falcon Drilling Rig #27 Moved onto this well on 9/30/17 and resumed drilling at 51 ft. KB. The 20" casing was set at 376 ft. on 10/2/17 and cemented to surface with 21 bbls returned. The 13 3/8" casing was set at 530 ft. on 10/23/17 and cemented to surface with 17 bbls returned. Coal was found from 482 ft. to 488 ft. Baskets were set at 472 ft. and 477 ft. The 9 5/8" casing was set at 1959 ft. on 10/5/17 and cemented to surface with 12 bbls returned. TD was at 6386 ft. on 10/7/17.
2663242 Primary Facility 2017-11-20 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #76 moved onto this well on 11/11/17 and resumed drilling at 6401 ft. MD on 11/18/17. Currently at 8504 ft. MD and drilling ahead.
2667432 Primary Facility 2017-12-05 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #76 KOP was at 6505 ft. EOB was at 8800 ft. MD (7614 ft. TVD). TD was at 16,878 ft. MD (7751 ft. TVD) on 11/26/17. The 5 1/2" casing was set at 16,848 ft. Estimated TOC at 5500.
2673450 Primary Facility 2017-12-15 Routine/Partial Inspection No Violations Noted On 12/15/17 a routine complete inspection of the Sandra Eakin well site was conducted in response to an operator request for review of a proposed minor modification to the site. The following was observed:

>>The operator proposed adding adding gravel to an area along one edge of the pad to level the gradient. A diversion culvert leading to a cross drain in the area will remain and be filled with large stone to allow it to continue functioning under the added gravel. The modification was approved.

>>Grease covered equipment and chemical storage containers were found off containment. No material was found to have reached the ground. Immediate remediation recommended.

>>Unmarked frack tanks reportedly containing fresh water without containment were observed on site. Frozen water around the valves corroborated the report of the water being fresh.
