The wells fraced are the 1H (Permit No. 37-125-27671), 2H (Permit No. 37-125-27672), 3H (Permit No. 37-125-27673), 4H (Permit No. 37-125-27674), 5H (Permit No. 37-125-27675), 7H (Permit No. 37-125-27676) and 8H (Permit No. 37-125-27686). Ten 500 bbl production tanks installed within stainless steel secondary containment. Frac tanks installed within secondary containment. Act 9 sign in place and installed at entry point to site. Erosion and Sedimentation Plan, Well Permits and PPC plan onsite in mailbox. Erosion and Sedimentation Best Management Practices installed and maintained. Vegetative growth established in previously disturbed areas. No apparent E/S issues.
2635657 Primary Facility 2017-09-13 Routine/Complete Inspection No Violations Noted On 9/13/2017, WQS Brian MacQuarrie onsite at unconventional well site. Seven wells in production with ten 500 bbl tanks installed within stainless steel secondary containment. Erosion and Sedimentation best management practices installed and maintained. Vegetation well established in previously disturbed areas. No apparent E/S issues.
2801293 Primary Facility 2018-11-08 Routine/Complete Inspection Violation(s) Noted On 11/8/2018 at approximately 0900, I conducted a routine inspection of the Walasik well site located in Jefferson Township, Washington County. I met with Angelo Albanese (Range Resources) to assess site conditions relative to permit termination. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • All wells on site were marked and appeared to be in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • GPUs were not within secondary containment but appeared to be in good condition. Wells at this site were put into production after October 2016. As a result, this constitutes a violation of 25 Pa Code 78a64a(b).
  • The site appeared to be stable and had greater than 70% vegetative growth.

The inspection of the project area confirmed that all requirements related to permanent stabilization and permit termination had been attained.

2812182 Primary Facility 2018-12-07 Routine/Complete Inspection Recurring Violations On 12/7/2018 at approximately 12:00, I conducted a site restoration inspection of the Walasik Well Site located in Jefferson Township, Washington County to assess site conditions relative to permit termination. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared in good condition.
  • GPUs on site were not within secondary containment but appeared in good condition. Wells at this site were put into production after October 2016. This is a continuing violation of 25 Pa Code 78a64a(b).
  • All wells on site were clearly marked and appeared to be in good condition.
  • The site appeared to be stable with greater than 70% vegetative cover.

Based on the inspection of the project area, it was determined that all requirements relating to permanent stabilization and permit termination had been attained.

I recommend termination of ESX12-125-0136.
