2483659 Primary Facility 2016-05-05 Incident- Response to Accident or Event In Compliance with Policy On Thursday May 5, 2016, I received notification of a release of brine water at the NV58HS Well Site located in Morris Township, Washington County. The release was discovered and reported on Wednesday May 4, 2016 and was estimated to involve approximately 30 to 35 gallons of brine water from a leaking dump valve in a GPU. The valve was repaired and the GPU and concrete slab were cleaned. The impacted gravel surrounding the concrete slab was excavated until conductivity probe readings identified unaffected material. The excavated material was placed into drums for off-site disposal. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.
2787417 Primary Facility 2018-10-05 Routine/Complete Inspection No Violations Noted On Friday October 5, 2018 I conducted a routine-complete inspection at the NV58 Well Site located in Morris Township, Greene County.

Gas well NV58GHS (125-24855) and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was tagged with identification.

No violations were identified during the inspection.
