On 10/14/15 I conducted a complaint inspection of the Mutnansky Unit in response to a complaint alleging that erosion had occurred at the site. At the time of inspection significant accelerated erosion was present. It was highly recommended that the issues present be addressed. I met with Mike Herman of Chevron at the site and he indicated that Chevron would be moving forward to address the issues at the site.

On 04/26/16 at approximately 1330 hours I conducted a routine complete inspection of the Mutnansky Unit. The Act 9 sign was in place and all required documents were present at the site. The accelerated erosion noted on 10/14/15 had not been repaired. It appears that a location part way up the pad slope was saturated and was the origin of the accelerated erosion. It is highly recommended that this be investigated.

2560512 Primary Facility 2017-01-27 Routine/Complete Inspection No Violations Noted On 01/27/17 at approximately 1300 hours I conducted a follow up inspection of the Mutnansky Unit. An NOV had been issued on 04/26/16 for 102.22(a). On the date of inspection the Act 9 sign was in place and all required documents were on site.
The majority of the site was well vegetated. The erosion noted in the NOV had been repaired, but erosion channels and what appeared to be tire tracks were present in the location of the repairs. BMPs were in place and sediment was not observed leaving the site. It is recommended that these issues be addressed once weather conditions permit.
2624012 Primary Facility 2017-08-07 Compliance Evaluation No Violations Noted The Act 9 sign was in place and all required documents were present at the site. Previously, water was seeping out of the pad slope causing erosion in the natural swale. The erosion was repaired, but earthmoving equipment tracks and ruts were still visible. The ruts were repaired and the area was seeded and mulched. The site overall was well vegetated with the exception of this area. The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.
