There were no ARD representatives on location at time of inspection. Well has a digital pressure gauge attached above swab valve, on the 5 1/2 x 9 5/8 annulus and on the production casing. Tubing pressure read 474 psi, casing pressure was 1228 and annulus pressure was 1537 psi at time of inspection. There appeared to be a spray pattern of an oily substance originating from a valve on the well head that extended out approximately 10-12 feet from the well head. Oily substance was also caked to the yellow guard rail that surrounds the wells on location.
2816398 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted On December 13, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. (Pad name). Monthly tank maintenance log was onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 6 producing wells located on this well pad. One plugged well was also on the pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 6 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.
