2489878 Primary Facility 2016-05-26 Routine/Complete Inspection No Violations Noted Routine Inspection; 5/17/2016.

I entered the location at 14:00 and began the inspection. At the time of the inspection, the Paulhamus Fredrick Unit 5H (081-20435), Paulhamus Fredrick Unit 6H (081-20551), and Paulhamus Fredrick Unit 7H (081-20852) were all in production. No erosion was noted on the site but the site lacked vegetative cover at several locations. It appears infiltration tests were conducted at several locations near the sedimentation basin.

The well site remains at its originally constructed size and has not been restored according to the PCSM (Post Construction Stormwater Management) plan in the ESCGP. The current ESCGP expired on 11/5/2015. The last well permits for this site were issued in 2012. The restoration assessment response from 8/13/2015 indicates the well site was restored but the Department continues to work with the operator to clarify implementation of well site restorations. Preliminary designs for restoration have been submitted.
