2582258 Primary Facility 2017-04-04 Routine/Complete Inspection No Violations Noted Routine inspection conducted at Anadarko's COP TRACT 289 Pad D on April 4, 2017. No one was on site during my inspection. There are six producing Marcellus wells on this pad. Natural gas leaving this pad enters the Pine Ck. No. 2 Water and Natural Gas Pipeline. Each well is properly identified with the well permit number and operator's contact information. Each well is connected to its own separate production unit. There are six production units on site. There are 4 (400)BBL produced fluids tanks located in secondary containment. There is approximately 2 inches of rain water within containment with a field screen reading of 68.0 uS/cm for conductivity. No issues observed with containment. The well pad remains its original size. No PCSMs have been installed. All E&S controls have been removed. According to the restoration appendices Anadarko submitted to the department in December 2016, this pad will get a permit upgrade in the 2017 calendar year. No violations noted.
2796553 Primary Facility 2018-10-30 Routine/Complete Inspection No Violations Noted On October 30, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating LLC. COP Tract 289 Pad D. Monthly tank maintenance log were onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 6 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 3 sand separators on site. There are 6 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.

No violations noted.
