2760024 Primary Facility 2018-08-03 Drilling/Alteration No Violations Noted The operator drilled the surface section to 370' and set and cemented 18 5/8" surface casing at 342' on 8/1/18. The operator reported no circulation or cementing issues. 42 bbls of cement was circulated to the surface by displacement. The operator is drilling out surface casing at the time of the inspection. Conduct site walk.
2760032 Primary Facility 2018-08-03 Drilling/Alteration No Violations Noted The well was spud on 7/24/18. The operator has set a 24" conductor pipe at 80'. The conductor pipe has been driven/drilled to depth.
2827705 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted On 01/09/19 at approximately 0835 hours I conducted a routine complete inspection of the site in response to a notification from Greylock that a Modular Above Ground Storage Structure was being constructed. As of the time of inspection the MASS had been constructed.

An area near the MASS showed signs of elevated conductivity compared to background (1.07mS vs 0.2mS). It is recommended the area be assessed for potential produced fluid impacts.

A water line between the MASS and well pad appeared to have been damaged by a vehicle (see photographs). It is recommended the line be pressure tested or repaired prior to use.
