2213077 Primary Facility 2013-10-08 Drilling/Alteration No Violations Noted CONDUCTOR SET. RIG ON SITE WILL SKID AND COMMENCE DRILLING OF TOP HOLE.
2231592 Primary Facility 2013-12-16 Routine/Complete Inspection No Violations Noted On 12/16/2013, I performed an inspection of EQT's Shipman well pad site. The site is located in Washington Twp., Greene County. The inspection was performed as a routine inspection during drilling.
2376712 Primary Facility 2015-06-04 Routine/Complete Inspection No Violations Noted On 6/4/15, I inspected the Shipman Pad as a routine inspection and made the following observations:
2809913 Primary Facility 2018-11-26 Incident- Response to Accident or Event In Compliance with Policy On 11/22/18 at approximately 0952 David Guidas of EQT reported an impaired water spill on the Shipman well pad.

On 11/26/18 at approximately 1200 hours I conducted an incident response inspection. I spoke via phone with David Guidas who indicated 10 gallons of produced fluid was spilled when a line containing impaired water was removed from a frac tank. Due to teardown activities at the site the secondary containment had a hole that allowed the fluid to reach the ground surface. The spill was remediated by SPSI. The spill has been addressed in compliance with DEP spill policy.
