2482075 Primary Facility 2016-04-26 Incident- Response to Accident or Event No Violations Noted On 4/26/2016, I performed an inspection of Rice Energy's Roger Parry 3H well site. On site during inspection was Dennis Furmanek (Rice Energy). The site is located in Aleppo Twp., Greene County. The inspection was is a routine inspection related to a reported spill by the operator.

The Act 9 sign was in place and legible.

The access road and well pad were in good condition.

The operator reported a spill of 30 gallons of brine to the ground due to a failure of a 2" fluid dump line at the Parry 3H GPU unit. The operator was on site excavating the impacted gravel from around the GPU unit. Field testing using a soil conductivity probe was being used to direct the intial remediation with confirmatory sampling to occur later. Operator has shut the well pad storm water drain valve closest to the spill location and is monitoring the water gathered in the sump for possible migration of brine through the well pad. A second inteceptor trench was dug near the spill site to gather impacted water from a prop
