2338187 Primary Facility 01/12/2015 Incident- Response to Accident or Event Violation(s) Noted I arrived onsite at 11:15 while Weatherford was onsite completing (perf. and hydraulic fracture) two wells on the pad. At the time of the inspection, they were under pressure on the 16H well (081-21476). After arriving onsite I met with Zachary Subra, Randi Mathew, and JJ Magyar (all Range). Sumps were installed off the west side of the well pad and field conductivity was greater than 20,000 umhos/cm (the meter's upper detection limit). In addition, water from inside the containment toward the south side of containment was also over 20,000 umhos/cm. Elevated conductivity was also noted outside containment in this area. The containment in question is secondary containment for the upright brine and freshwater tanks. Based on the initial evidence, it appears brine from the upright tanks was released to the containment which subsequently released to the pad surface/ soils. Sample 3440216 was collected at 11:50 from a sump off the west side of the well pad (preliminary lab results attached). Impacted soils/sumps were being excavated. Vios-->16H

At the time of the previous inspection; impacted soils and snow were removed to the extent possible and an intercept trench was installed and periodically vacuumed as needed. Conditions onsite remained frozen until fairly recently.

I entered the site at 09:53 with Jared Rutt (DEP). The Laurel Hill B Unit 1H (081-21212), Laurel Hill 12H (081-21410), Laurel Hill 13H (081-21411), Laurel Hill 14H (081-21412), Laurel Hill 15H (081-21475), and Laurel Hill 16H (081-21476) were in production. The well site was recently restored and the east and west sides of the pad were reduced in size and vegetated according to the restoration extension approved 6/30/2016. The disturbed areas have been seeded and stabilized. No Violations Noted.

I entered the site at 12:50. PCSM has been implemented and the size of the impervious pad surface has been reduced. The site is well vegetated but not yet permanently stabilized. Some scouring of gravel was noted at the top of the fill slope above the infiltration basin. I recommend monitoring and re-grading as needed to minimize concentrated flow unto the basin.

2642203 Primary Facility 2017-09-29 Routine/Complete Inspection No Violations Noted Weather was 50° and sunny at time of inspection. There were no Range resources company representatives on location at time of inspection. Tested 13 3/8 x 20 annulus vent. 0% combustible gas was detected. There was no 9 5/8 x 13 3/8 annulus vent observed. 5 1/2 x 9 5/8 annulus vent was plumbed to a poly tank. Well had a 0 to 5000 PSI gauge attached above the swab valve. Gauge was reading approximately 1200 psi a time of inspection.

At the time of the inspection, 5 wells onsite were in production. The PCSM was implemented for the site and the footprint if the well pad surface was reduced. The site is permanently stabilized and temporary erosion and sedimentation controls were removed. I recommend approving the Notice of Termination and Well Site Restoration Report submissions. No violations noted.
