2315070 Primary Facility 10/14/2014 Routine/Complete Inspection No Violations Noted At the time of the inspection the Nabors M-39 was onsite and had just moved to the 4H (081-21357). After arriving onsite I spoke with the company man Mr. Coggins and we walked the site. Overall, the site housekeeping had room for improvement. Lime stabilized diesel-based mud cuttings were noted outside containment but on liner. In addition, petroleum based additives and a friction reducer were spilled on the well pad containment. I asked the company man to clean-up both issues ASAP due to the forecast rainfall for the following day. No Violations Noted.
2327368 Primary Facility 12/02/2014 Incident- Response to Accident or Event No Violations Noted On 12/1/2014 Range Resources reported a <1 bbl release of flowback from a flow line to the production tanks. The release sprayed flowback on the tanks and adjacent soils and vegetation. The soils were being excavated on 12/2/2014 (at the time of the inspection). Much of the impacted soil was excavated and covered with plastic prior to sleet/snow on 12/2/2014. Overall, field (soil) conductivity indicated a few areas elevated above 1,000 umhos/cm. More excavation is planned as weather permits. Violations cited to the 1H (081-21446) well.

2331542 Primary Facility 12/18/2014 Compliance Evaluation No Violations Noted This inspection is in response to the 12/2/2014 inspection which cited violations for a brine release (spray) to the ground and adjacent vegetation. The majority of the excavation was complete on 12/2/2014 but additional remediation was delayed due to snow and ice accumulation. Soil conductivity was 100-200 umhos/cm at several locations which were previously excavated. Field conductivity (water) in two puddles was 605 umhos/cm and 713 umhos/cm respectively. Based on the field data the violations cited on 12/2/2014 are considered resolved. No new violations.
2331658 Primary Facility 12/22/2014 Incident- Response to Accident or Event No Violations Noted On 12/22/2014, Randi Mathew (Range) reported a release of flowback/produced water from the 2H (081-21447)T-Pak caused when the dump valve washed-out. The volume of the release was estimated at less than one barrel at the time of the notification. Onsite, a mini-excavator was being used the begin excavation on the south side of the 2H T-Pak. Evidence of the spill was visible on both the west, south, and east sides of the production unit flowing south down the slope. Field condictivity in the excavation was >20,000 umhos/cm (over the detection limit). Soil conductivity suggested the spill did not reach the drainage swale or the infiltration basin. Based on the extent of the spill, confirmation samples are needed to determine compliance. Please also be sure to delineate the extent including under the production unit. Violations Noted to the 2H (081-21447) well.
2333288 Primary Facility 12/30/2014 Incident- Response to Accident or Event Violation(s) Noted On 12/29/2014, Zachary Subra (Range) reported a release of produced water from the 4H (081-21357) T-Pak caused when the dump valve washed-out. The volume of the release was estimated at less than one barrel at the time of the notification. On 12/30/2014, Thomas McNerney (DEP) and I arrived onsite a met with Zach Subra and Randi Mathew (Range). Evidence of the spill was visible on both the south and east sides of the production unit flowing south down the slope. Some of the released brine was collected in a sump south of the 4H production unit. Sample 3440213 was collected at 10:25 from the sump. Conductivity was 435 uhmos/cm in the infiltration basin southwest of the production units. The volume of brine released to the ground does appear to be greater than 42 gallons. Based on the extent of the spill, confirmation samples are needed to determine compliance. As discussed onsite, the response for both the 12/22/2014 and 12/30/2014 NOV's as well as the final remediation report can be combined. Violations cited to the 4H (081-21357) well

Well was in production. Well had a 0 to 5K PSI analog pressure gauge reading approximately 400 PSI on the production tubing. Well had a 0to 10K PSI analog pressure gauge reading approximately 500 PSI on the production casing. Well has 2 visible annulus vents plumbed to surface. The 9x5 annulus vent was plumbed with a 2' line to the 9x5 annulus vents on location and plumbed into a poly tank. The 13x9 had 0% combustible gas present at time of inspection. 0% combustable gas was present anywhere around the well at time of inspection. 77% LEL and 4% methane was detected at the polytank. No violations at time of inspection.
