Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2113476 Primary Facility 10/25/2012 Routine/Complete Inspection No Violations Noted 1025-1030H cellars, conductor, and mousehole set. No E&S issues
2467341 Primary Facility 2016-03-22 Routine/Complete Inspection No Violations Noted
2780731 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. The emergency response plan and inspection logs were onsite.

There are 6 tagged Marcellus wells staged for production. There are 6 GPUs onsite and in secondary containment. There are also 4 produced water tanks in secondary containment and 1 double walled produced water tank not in additional secondary containment.

A brush barrier was constructed as indicated on PCSM/SR plans. At the time of inspection compost filter sock also remained around the site and appears to have been seeded. Areas around the pad are well vegetated and no E&S issues were observed.

The tag for well 1026H appears to have fallen off and been reaffixed under a U-bolt (Photo #8). The department recommends affixing this tag in a more permanent manner.

No violations noted.
