2581143 Primary Facility 2017-04-11 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site overall was well vegetated but there were locations along the access road that could use additional seeding. Small erosion rills were observed in several locations throughout the site. An erosion channel was observed in the swale between the AST pad. The wells had previously been spud, but additional drilling has not yet occurred.
2611995 Primary Facility 2017-06-26 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site overall was well vegetated, but some erosion was observed in the swale between the AST pad and the access road. Six SPUD wells were present at the site.
2638231 Primary Facility 2017-09-20 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated. A small erosion rill was observed between the AST pad and stockpile. The wells on the site had been previously SPUD.
2658476 Primary Facility 2017-11-06 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and stable. The wells on site had been SPUD, but not drilled to TD. The wells are currently on inactive status. There were no violations observed at the time of inspection.
2686497 Primary Facility 2018-01-19 Routine/Complete Inspection No Violations Noted Inactive Status was approved (6/17/2016).Pending Permits issue being address now.
2752612 Primary Facility 2018-07-17 Drilling/Alteration No Violations Noted Highlands Rig #14 currently TTR @ 6735'. All seven wells on location are in stages of air rig build into the curve with 8 3/4" hole, operator will then load hole with mud and move off air. Fluid rig will follow to complete lateral. Location having issues with gas production above mine and are using cement pill plugs before drilling into mine to reduce or eliminate gas migration from uphole. On site inspection found no apparent violations. NOVIO
2761385 Primary Facility 2018-08-07 Drilling/Alteration No Violations Noted Operator has set 9 5/8" csg and drilled 8 3/4" hole into the kick, prepped well for triple to move on location to finish lateral. On site inspection showed no apparent violations. NOVIO
2845975 Primary Facility 2019-02-15 Compliance Evaluation No Violations Noted On 11/06/18 an Oil based mud spill had occurred at the site and an NOV was issued.

On 02/15/19 at approximately 1015 hours I conducted a CEI at the site. The Act 9 sign was in place and all required documents were present at the site. The impacted soils had been remediated and all contaminated equipment had been cleaned and removed from the site. The violations have been corrected and there were no violations observed at the time of inspection.
