On 06/17/16 at approximately 1000 hours I met with Curtis Small and Nathan Isler at the Hoehn, Howe, Hall Hogsett Unit to discuss third party impacts to their restoration work.

Chevron has documented on multiple occasions third party impacts to the site restoration efforts. The purpose of the meeting was to meet with Chevron representatives to witness the damage done to their most recent restoration attempt. Previous damage had been repaired and hydroseeded and the following day a third party that is not the land owner nor had permission to access the land owner’s property impacted the repaired area with an all-terrain/off road type vehicle.

After discussions it was decided that Chevron is going to compile and submit a document for the Department’s review demonstrating the multiple attempts to restore and repair the third party impacts that have prevented them from submitting for an NOT.

2565505 Primary Facility 2017-02-27 Routine/Complete Inspection No Violations Noted Act 9 signs for the Howe, Hall Hogsett, and Hoehn well pads were in place. All required documents were present at the site. The site was well vegetated and there were no signs of erosion observed. Locations of previous third party impacts had been repaired.
2600027 Primary Facility 2017-06-02 Administrative/File Review No Violations Noted An administrative inspection was conducted in response to Chevron submitting a well site restoration report for the Hoehn well pad. The site had previously been inspected and was found to be restored.
2636675 Primary Facility 2017-08-11 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal by Chevron. The Act 9 sign was in place and all required documents were present at the site. The ESCGP covers the Hall Hogsett, Howe, and Hoehn well pads. The well pads and production equipment appeared to be in good condition. Previous third party impacts had been documented at the site. Chevron had repaired the damage caused by third party actors. The site was well vegetated and there were no signs of erosion observed at the time of inspection. The site meets the requirements for permit termination. The NOT is approved
