2274777 Primary Facility 05/29/2014 Routine/Complete Inspection No Violations Noted On 5/29/2014, the Department conducted an inspection of Chevron Appalachia’s Skovran Unit 37, 38, 39, 40 H well site in Redstone Twp., Fayette County.
2462123 Primary Facility 2016-02-25 Routine/Partial Inspection No Violations Noted

On 02/15/16 at approximately 1300 hours I conducted a routine partial inspection of the Skovran Unit in order to meet with Curtis Small of Chevron. The Act 9 sign was in place. All required documents were present at the site.

Chevron contacted me to report that a slide had occurred on the site on the edge of a sediment trap. I met with Small to discuss the incident and repairing the slide. The slide has not left the LOD, nor has it impacted waters of the Commonwealth. Due to the location of the slide and the current and expected field conditions, it was agreed upon that the slide will be repaired when dry conditions permit.

Chevron is currently researching how to prevent a future slide and is considering a drainage system for the slope. Chevron is going to contact DEP O&G engineering section staff to discuss if modifications to the permit will be necessary.

2564702 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. A slide had previously occurred on the site and had been repaired (see photographs). Portions of the site appeared to have been recently seeded and mulched. There were no signs of erosion observed. The equipment appeared to be in good condition. There were no violations observed at the time of inspection.
2658214 Primary Facility 2017-10-26 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal by Chevron. The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and met and exceeded uniform 70% vegetative cover. There were no signs or erosion observed. There were no violations observed. The site meets the requirements for permit termination. The NOT is approved.
2839250 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted This well is tagged and temporarily shut in. The well head casing, piping, production set equipment, containment dike and fluid collection tank look ok.
