2636518 Primary Facility 2017-09-08 Routine/Complete Inspection No Violations Noted Routine inspection of COP TRACT 344 Pads A&B on September 8, 2017. No one was onsite during my inspection. Both pads are covered under the same ESCGP and they have been upgraded to an ESCGP2.

COP TRACT 344 Pad A Pad A has a total of 6 Marcellus wells on it. Four of the six are producing. Wells 035-21220 and 035-21223 have been plugged and are represented with a monument. I recommend updating the well tags on each monument to reflect Alta's emergency response information. Each well tag has Anadarko's name and contact phone number on it (see photos). Attached to each producing well head is a line from a 220 gallon tote of foam located on secondary containment. Each well is connected to its own production unit. There are 3 dehydration units onsite. In addition, there are (4) 400 BBL produced fluids tanks located in secondary containment. There appears to be no tears, rips, or leaks from the containment. Just a trace of water was found within this containment. This pad appears stable. All E& S controls have be

2719861 Primary Facility 2018-04-10 Routine/Complete Inspection No Violations Noted The weather is 39 Deg. F and cloudy at the time of inspection. Routine inspection of Alta Resources, LLC., C.O.P. Tract 344 Pad, A-1001H well on April 10, 2018. There are 6 wells on this pad.

Well # A-1001H is active and producing. Intermediate by surface (9 5/8¿ x 13 3/8¿) annulus vent is shut in to pressure monitor, with pressure reading 1100 PSI. The production xmas tree top pressure reading is 931 PSI, casing pressure is 978 PSI. 0% combustible gas was detected in the area immediately surrounding the well. No violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2756680 Primary Facility 2018-07-20 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at ARD Operating's COP Tract 344 Pad A on July 20, 2018. This pad has a total of 6 Marcellus wells on it. Four of the six are producing. Wells 035-21220 and 035-21223 have been plugged and are represented with a monument. Each monument is labeled with Anadarko's name, well permit number, and contact phone number.

Attached to each producing well head is a line from a 220 gallon tote of foam located on secondary containment. In addition, a tote of methanol is stationed in secondary containment near the well head and is connected to each well. Each well is connected to its own production unit. There are 3 dehydration units onsite. In addition, there are (4) 400 BBL produced fluids tanks located in secondary containment. There appears to be no rips or leaks from the containment. Just a trace of water was found within this containment. This pad appears stable. All E& S controls have been removed. A vegetative filter strip is in place per

2820830 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted On December 26, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 344 Pad A. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 4 producing wells and 2 plugged wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 4 GPU’s and 4 steel vertical production fluids tanks onsite. The production fluid tanks have some corrosion and staining (See photo 1). Both GPUs and production fluid tanks were stationed in secondary containment. Foam and methanol are located on the pad in secondary containment.
