2680886 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the EQT Phoenix Pad E located in Duncan Twp, Tioga Co. on 1/9/2018 @1130 hours. The weather was 33 degrees partly cloudy. Winds WNW 15 mph. There was no activity onsite during the inspection.

Three wells were observed in production onsite, the Phoenix 590628, 590809 and 590806. Additionally, there are three active well permits, the Phoenix 590811, 590812 and 590813 that were issued 2/17/2017, but have not been drilled yet. An aboveground storage tank (AST) labeled "freshwater only" was observed onsite. A temporary water line is connected to it.

Adjacent to the pad is the Pad E impoundment. The impoundment appeared to have adequate free board and was frozen over and snow covered at the time of the inspection.

No violations are noted at this time. Please address any deficiencies noted. I departed the site @1300 hours.

2743617 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the EQT Phoenix Pad E located in Duncan Twp, Tioga Co. on 6/19/2018 @1200 hours. The weather was sunny, 78 degrees, 45% humidity, Winds W 12 mph. There was no activity onsite during the inspection.

The site consists of the access road, impoundment and well pad. Three wells were observed in production onsite, the Phoenix 590628, 590809 and 590806. Additionally, there are three active well permits, the Phoenix 590811, 590812 and 590813 that were issued 12/11/2017, but have not been drilled.

No violations are noted at this time. Please address any deficiencies noted. I departed the site @1300 hours.

2799248 Primary Facility 2018-11-06 Routine/Complete Inspection No Violations Noted The DEP conducted a Routine inspection of the EQT - Phoenix Pad E located in Duncan Twp, Tioga Co. on 11/6/2018 @1245 hours. The weather was 52 degrees, rain, winds NNE13mph. No company personnel were present onsite during this inspection.

The site consists of the access road, impoundment and well pad. Three wells were observed in production onsite, the Phoenix 590628, 590809 and 590806. Additionally, there are three active well permits, the Phoenix 590811, 590812 and 590813 that were issued 12/11/2017, but have not been drilled. These permits will expire 12/11/2018.

Please address any deficiencies noted and contact me with any questions. I departed the site at 1300 hours.
