It was reported that an estimated 10 gallons of production fluid were released to the well pad surface on 5/7/15. A slug/burp of production fluid exited the hatch on the top of a production fluid tank in the form of a mist as a Cabot well tender was in the process of performing maintenance on the Grosvenor 1H well. The well tender was adding soap sticks into the well to decease the hydrostatic pressure of the water in the well.

2484340 Primary Facility 2016-05-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:15 p.m., the cellar was full of water with no bubbling notice, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 522psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:40 p.m.

2487885 Primary Facility 2016-05-26 Site Restoration No Violations Noted Inspection of the Grosvenor D P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.
