1793866 Primary Facility 04/17/2009 Drilling/Alteration Violation(s) Noted Based on records, well was completed on 6/1/01 to a depth of 5,640 ft. from grade. On 3/22/02, WRAC submitted and approved by the DEP. During the inspection, well was located on hillside of agicultural field along Apple Arches Road. The well shows an outside flush 13" conductor pipe with an inside 7" pipe. The annulus between the pipes appears grouted. A wire screen was welded inside the 7" inch pipe. No well tag present on the well. Based on records, a plug report could not be located at the DEP. The site has not been restored. Mounds of unconsolidated material remain at the site.
1819792 Primary Facility 08/04/2009 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Meeting held with Mid East Oil (Lance Cassidy) and DEP (Ed Renwick, Paul Wyrwas, Chad Eisenman and April Wieland) to review 4/17/09 inspection. According to Mid East, well was drilled to TD depth but was never stimulated and was not properly abandoned. Pits were restored and mound of soil was remaining top soil which is protecting the well from agricultural activity. DEP requested that Mid East install a vent pipe over the well to prevent material from entering the well. As indicated in the DEP meeting, Mid East is in the process of abandoning this well according to Oil and Gas regulations. No new violation recorded since the 4/17/09 inspection. However, these violations have not been corrected by Mid East Oil.
