The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The access road was installed and well maintained. The All slopes were stabilized and well vegetated. The perimeter BMPs should be removed from the site if all construction activity has been completed and a well restoration report submitted to the Department after completion of drilling. No violations were cited during this inspection. - MRN

2645868 Primary Facility 2017-10-10 Routine/Complete Inspection No Violations Noted On 10/10/17 at 1325 hrs I conducted an inspection of the Yonkin Unit 1 well site. No one was present at the site. It was sunny and 74 degrees F.

The Yonkin 1 well was drilled by Tenaska in 2011; the target formation listed on the well plat is the Bradford Group. The well, pump jack, and production equipment are present at the site. The well was tagged. According to DEP records, the well is not in production and is waiting on a pipeline connection. The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a).

A Well Site Restoration Report (WSRR) was not found on file. A WSRR should be submitted as per 25 Pa Code Section 78.65(3), which states, in part: "Within 60 days after the restoration of the well site, the operator shall submit a well site restoration report to the Department." It is encouraged that the WSRR be submitted electronically.

No violations were cited during the inspection. I departed the location at 1400 hrs. TLT
