2008728 Primary Facility 10/05/2011 Routine/Complete Inspection Pending On Oct. 5, 2011 an inspection was conducted at the Reed Run Keating Pad A 2522 well site. The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The last inspection conducted on behalf of PGE was on 9/30/11. Some perimeter BMPs had been removed and some were still installed. The site was stable, drilling was completed and the fracing process was currently being completed. Several puddles on the site showed elevated reading for specific conductivity. A sample was collected and submitted for analysis. Violations are pending the results of the analysis. A Well Restoration report was submitted for this site and is pending.
2015114 Primary Facility 10/31/2011 Follow-up Inspection Violation(s) Noted On Oct. 5, 2011 an inspection was conducted at the Reed Run Keating Summit Pad A 2522 well site. A violation noted during a 11/16/2010 inspection (1928105) for 78.65(3) - Failure to submit or submitting an inadequate well site restoration report within 60 days of restoration of the well site has been corrected and resolved. The sample results for the spill noted during the Oct. 5, 2011 inspection (2008728) resulted in new violations. The spill resulted in violations of 35 PS 691.402 - Potential to pollute waters of the Commonwealth, 35 6018.301 SWMA and 78.56(a) - pits and tanks not constructed with sufficient capacity to contain pollutional substances. The failure to notify the Departmant of the spill resulted is a violation of 25 PA Code 99.31 (a) - failure to report a pollutional incident. The Department has received photo documentation that the area has be remediated, but additional information as to how the cleanup was undertaken has been requested. - MRN
2026861 Primary Facility 12/15/2011 Follow-up Inspection No Violations Noted On Dec. 15, 2011 an inpsection was conducted to follow up on violation discovered during an On Oct. 5, 2011 inspection at the Reed Run Keating Summit Pad A 2522 well site. The violation were cited in an Oct 31, 2011 (2015114) inspection report after receiving the laboratory results. The Department has received photo documentation that the area has be remediated, and reading of the puddles on the pad and the associated ditches during todays inspection did not exibit elevated reading for specific conductivity. Based on today's inpsection and information previously submitted by PGE to the Department, the violation noted above are being resolved. No new violation were cited during this inspection. MRN

2648626 Primary Facility 2017-10-17 Routine/Complete Inspection No Violations Noted On 10/17/17 at 1036 hrs I conducted an inspection at the Reed Run Keating Pad A well site. It was sunny and 37 degrees F. No one was present during my inspection.

The wells have been drilled, tagged and equipped for production. The production equipment has built-in containment, which appeared to be in good condition. Three production tanks are staged in containment. The containment held several inches of water; field measured specific conductance was 2.72 mS/cm. No staining or evidence of spills was noted on the pad surface.

The freshwater impoundment located north of the well site was observed. The access road entrance off of Card Creek Road was gated. The perimeter of the impoundment site is fenced, however a portion of the fence has fallen down and should be re-erected. It appeared that the impoundment held about a foot of water. The liner was patched in numerous places and several holes were observed. The holes were above the current water level, although a few would be below the water if the impoundm

The wellhead assembly all fittings and flow lines were tested for gas. The results of this testing was no gas was detected on the wellhead assembly or the well area.
