The site was snow-covered and vegetative cover and stability could not be assessed. The pad has been permitted to be partially restored upon restoration. The pad has not yet been restored.

Post Construction Stormwater Management (PCSM) is handled by three infiltration basins. Basin #2 was holding water and ice at the time of inspection.

A 2-year restoration extension was approved October 11, 2017 and expires October 11, 2019.

2810467 Primary Facility 2018-12-04 Site Restoration No Violations Noted A Notice of Termination (NOT) was received by the department on November 26, 2018 which promoted this inspection.

One well is staged for production with one GPU and sand separator. Two steel produced water tanks are present and are moderately corroded. Steel containment with spray-in liner is present and in good condition. Several labeled containers of methanol are present and appear to be in good condition.

Post Construction Stormwater Management (PCSM) is handled by 3 infiltration areas. All three have been constructed as permitted and appear to be functioning as designed. Infiltration area #1 is retaining some water. The pad has been permitted to remain at full size. Pad side slopes, topsoil stockpile, PCSMs, and the remainder of the area within the LOD have been stabilized are well-vegetated with greater than 70% vegetation.

Site is stable. No E&S issues noted. No violations noted.
