2568805 Primary Facility 2017-03-08 Routine/Complete Inspection No Violations Noted On Monday, March 6, 2017, I completed a routine complete inspection of the Phillips Gas Well Pad located in Morris Township, Greene County.

The Act 9 Sign was clearly legible at the site entrance. The ESCGP-1, ESX09-059-0032, governing earth work at the site was terminated sometime around late 2015 to early 2016. Given that the ESCGP-1 was terminated, the operator is not required to keep the permit or the Erosion and Sedimentation Plan on-site. The Emergency Response Plan and PPC Plan were available on-site, along with well permits, inspection reports and some other loose paperwork. The operator may consider keeping an As-Built / Interim Restoration drawing on-site. The ESCGP-1 predates the requirement for Post Construction Stormwater Management Plans and controls.

2810606 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted On Tuesday December 4, 2018 I conducted a routine-complete inspection at the Phillips Well Site located in Morris Township, Greene County.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was tagged with identification.

No violations were identified during the inspection.
