2017962 Primary Facility 11/02/2011 Routine/Complete Inspection No Violations Noted Site construction had been completed, driling had not commenced at the time of the inspection. Noted that the site had been cinstructed in a maner that there was an area near the well stake which was about 2 feet lower tha the remainder of the pad. This would result in an accumulation of runoff flow from the well pad and concentration of the runoff flowing off the pad. It was noted that this may have already occurred and that the operator had maintained the eisting erosion controls as neccesary as a result of this runoff flow. Touched base with MDS Enegry via phone regarding this site and the recommendation that some action be taken to bring this low level area up to meet the remainder of the well pad. Drill pit onsite, it contained fluids and it had more than the reqired two feet of lined freeboard.
2018459 Primary Facility 11/09/2011 Drilling/Alteration No Violations Noted First Class Energy Rig#1 Schramm drilled to 7000' TD at time of inspection. Pit lined and freeboard maintained.
2026185 Primary Facility 12/13/2011 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Drill pit located onsite and it contained drill cuttings and drill fluids. Drill pit had more than the required two feet of lined freeboard. Access road and erosion controls had been implented and maintained properly. There was no observed accelerated erosion or sedimentation related to the access road or site.
2130020 Primary Facility 12/21/2012 Site Restoration No Violations Noted Site was drilled and appeared not to be equipped for production. Site had been re-graded and seeded. Vegetative cover appeared to be uniform and approaching 70 percent. Erosion and sedimentation controls appeared to be properly installed. No erosion or sedimentation issues were evident.

Well-site ahd been appropriately restored and the was no evidence observed of accelerated erosion or sedimentation in relation to the site or access road.
