The Department recommends that the operator monitor the site to insure 70% uniform vegetative cover is achieved. There was no accelerated erosion or sedimentation observed in relation to the access road or well-site. The Department recommends that the operator implement either a swale with erosion control matting which will grow into a grass lined swale or a rock stabilized outlet associated with the culvert conveying runoff under the access road as it approaches the well head. Additionally it was noted that a freshwater impoundment located above the well pad on a separate access road that is also associated with well 005-30164. This freshawater impoundment is associated with and was constructed for well development activities for the 005-30652 Matthew Anthony 3M-86 well.

Well-site had been regraded and vegetation appeared to have begun to take hold. An impoundment was located in conjucntion with this well that as per the operator contained freshwater, precipitation and snow melt. This impoundment had more than the required two feet of lined freeboard.
