1007178902 2015-04-01 On-site inspection to follow up on the recent oil spill that occurred on 03/27/2015 and compliance notice #1425400636 follow up inspection. On this day I was on site with Division personnel from the Engineering division Andy Thomas and Jason Simmerman along with Hydrologist Rick Tripple. Upon inspection it was observed that Littman Excavating was excavating the Westerly slope below the well pad tank battery with our initial assessment finding the excavation to be in depth of approximately 20' into the shale layer and exposing the subsurface bedrock, due to slop stability issues further excavation deeper into the well pad was ceased. The company will now remove the 4-400 barrel production tanks and begin to remediate the affected material from under the tank pad. Compliance Notice
1217302780 2013-07-18 Notification was received from the company man on site that the current well bore drilling segment is complete with a total depth drilled of 8700'. They are currently logging the well bore and then will start running casing. I have discussed with the company man on site the fact that they only want to cement the 9.625 to around 4,400' on the backside and are leaving the Devonian Shale open.

1368549354 2013-05-14 My on site inspection found the site pad construction 90% complete and the top hole rig on site. The company man stated they are to spud tomorrow 05/15/2013. I confirmed the well stake to the correct GPS point on the permit and will enter the gps readings once the well is spudded and the site entry point is complete.

1373303475 2013-07-08 Inspection of the drill site found the company rigging up the BOP on the 13.375 casing string. I also received records from the 13.375 casing string set on 07/07/2013.
1374851968 2013-07-26 On site inspection to witness the setting of the bottom hole strata test plug and kick plug. The well bore true vertical depth drilled was 10,930'. 2 plugs were set through 5 drill pipe with 1000' of 2.875 tubing hanging below. The plugs were displaced with oil based drilling mud with a 3 barrel plug between the cement and drill mud.
1375761398 2013-08-05 Status check inspection of the production casing cement job, The well bore total depth is 17,107' with 17,057' of 5.5 23# production casing ran along with 128 casing centralizers and a casing shoe and float collar with a 44'10 casing shoe track, Baker Hughes cement services cemented the casing with a lead and tail cement mix and a total of 2151 sacks of class H cement, a 100 barrel pre flush was ran prior to cementing. The casing was pressure tested to 3700 psi upon the landing of the cement plug.
-1380334583 2013-07-01 Inspection found the drilling rig Sidewinder Drilling Rig # 62 finishing up the air body annular test and getting ready to drill out the casing shoe of the 13.375 casing string. The Air body and casing string was tested to 850 psi. Records were received from the 713' of 18.625 casing set on 06/30/13.
1421775399 2015-01-20 Preliminary Restoration status check inspection found the well drilling and stimulation process is now complete and the well in production. There is 4- 400 barrel production tanks located in an plastic lined metal liner with 2 of the tanks present related to the said well along with 2 in line heaters on location and a positive pressure flair stack burning the excess gases of the production tanks. Preliminary Restoration was passed on this day.
1427485377 2015-03-27 On this day I received notification from the company @ 7:52 am in regards to a reported product spill of 15 barrels of produced oil into a ditchline off the well pad. Upon my arrival @ 9:15 am it was observed that a Ohio Oil Gathering tractor trailer was on location loading product to be transported out of the 6-400 barrel production tanks and during the loading process product was lost onto the earth surface from a loading ventilation line that was open to the atmosphere and not in containment. Upon investigation of the affected area it was observed that product had traveled a measured distance of 670' from the point of contact where the product exited the vapor ventilation line and a exact product amount has now been reported of 30 barrels or 1,260 gallons. XTO had representatives on site and has commissioned HULL Consulting Environmental to conduct soil sampling analysis along with Littman Excavating to conduct the cleanup process. Representatives from Regulatory Agencies from the State of Ohio we also present as listed-ODNR Oil &Gas- OEPA -ODNR Wildlife- Ohio PUCO -Monroe County EMA . Moving foreword with the clean up and recovery Littman is working with oil absorbent spill rags and oil boomers recovering the product and protecting the tributary stream know as Blair Run, they have also mobilized a small excavator and are working to recover the contaminated soil along the private driveway adjacent to and below the well pad as to where the majority of the product was gathering. Instructions are as follows - Recover product and Remediate affected area- ODNR Monroe Co Inspector -Bill Stolarik 740-507-6592.
1427734645 2015-03-28 My inspection today finds the well operator is continuing to remediate the condensate spill that took place the previous day. Litman Excavating, Hull Environmental, BBU, and XTO employees are cleaning the stream bank and removing all contaminated soil within the limits of disturbance. Three underflow dams were constructed in the wet weather stream. By the end of the day an eighty-seven foot long five feet deep section of Township Road 910 was removed due to contamination and work to rebuild the road had began.
1427919113 2015-04-01 Status check inspection on the current drilling status and well control issue found the well control blow out preventer in the closed position and the company preparing to circulate the gas pocket out of the 8.750 well bore with 1350 psi found on the annular side of the drill string at surface. The current drill mud weight is 13.7#.
1429232967 2015-04-16 On this day a meeting occurred at the XTO Energy office at Bellaire Ohio in regards to the recent oil spill that occurred on 03/27/2015 and compliance notice #1425400636 issued. In attendance were staff members from XTO Energy : EnLink: Hull & Associates: OEPA:ODNR. During the course of the meeting a Draft Memorandum summary of the completed activates related to the Condensate spill was present to all parties in attendance. Include in the Memorandum was a interim monitoring plan. In a separate discussion post meeting Regional Supervisor Brent Bear Discussed with XTO the language of the violation and the procedure in which will allow the violation to be considered resolved. ODNR Engineering Technician Andy Thomas -Regional Supervisor Brent Bear and myself proceeded to the well site following the meeting and reviewed the clean up remediation process and briefed Supervisor Bear on the location layout.
-1537338678 2015-03-29 Follow up inspection on the recent product release recovery and clean up found Littman Excavating on site excavating the slope between the tank battery and adjacent road way with the road way being excavated and replaced to a depth of 5'. HULL Environmental is on location conducting soil testing measuring the VOC's in PPM levels. All soil is to be removed until no readings are obtained. Several siphon dams were erected in the defined ditchline in the affected area below the roadway.

1813755445 2013-06-27 Status check inspection of the current drilling progress found Sidewinder Drilling rig # 62 has rigged up on the hole to complete the well bore drilling procedure with the rig currently running in the hole with a 22 air hammer to drill out from under the surface casing. The company will test the surface casing string once again by closing their air body.

-371363452 2013-05-31 On site inspection to witness the running and setting of the surface casing, the original permit conditions were modified with approval of the permitting section of the Division due to a unknown mine void depth. On this day 303' of 24 surface pipe was set and cemented to surface with 60 barrels of cement returns to surface. The casing tally and cement tickets were received on this day.
-489598037 2013-10-21 Notification was received from the company man on site over the past weekend that they had reached their total drilling depth at 11260' and are running and cementing the 5.5 production casing, upon inspection on this day the well bore drilling is complete and the rig is skidding to 3411124381-0000.
-592103099 2015-04-15 On-site inspection to follow up on the recent oil spill that occurred on 03/27/2015 and compliance notice #1425400636 follow up inspection. Upon inspection it was observed at the production fluid storage area that the 4-400 barrel capacity tanks have been removed and the underlying area tested and remediated accordingly. With the vent line that was the initial contributing factor in the spill being reinstalled to allow the product to be discharged back into the containment area, the company has also installed a fluid protection valve to also protect the system from discharging fluid by a automatic shutdown system in the event that fluid should be present in the system. The Divisions Engineering section has been addressing the remediation of the affected area below the tank battery where extensive excavation has taken place.

661012405 2015-03-31 On-site inspection to follow up on the recent oil spill that occurred on 03/27/2015. On this day it was observed that Littman Excavation was on site removing earthen product to an approximate depth of 10' in the affected area between the production tanks and the lower roadway. A assessment was done over the entire affected area of the spill with Trevor Irwin with the Ohio EPA and representatives of Hull Environmental and XTO Energy's EHS Coordinator, VOC readings were taken to determine the amount of product still present in attempt to determine the course the product flowed and the total affected area. Per the EPA -3 siphon dams have been built in the defined wet weather stream bed located on the Westerly slope where the product traveled off site. Littman Excavating employees continue to conduct recovery operations of the fluid migrating to surface and recover the fluid being captures by the dams.
722611437 2013-07-15 Inspection found the current drilling depth to be 7054' using a 12.250 drill bit. The projected total depth for this current well bore section to be drilled is 8700'.

-898779205 2015-04-06 On-site inspection to follow up on the recent oil spill that occurred on 03/27/2015 and compliance notice #1425400636 follow up inspection. On this day I received a phone call from Todd Snyder who is the HSE coordinator for XTO Energy , in which we discussed the quantity of product initially spilled on the day of the event, it is now being reported that a amount of 46 barrels of product has been lost. These estimations have been generated through the computer system that monitors the well flow and tank gauge levels. My on site inspection on this day has found the company currently backfilling the Westerly slope area that had initially been excavated and removed due to contamination from the spill event. Also reported during my inspection the company shall remove the production tanks on 04/06/2015 and evaluate the affected area under the tanks.
919058422 2013-07-11 Status check inspection found the current drilling depth to be 4312' using a 12.5 hammer bit, with the company projecting to run on air to approximately 6800' to the top of the salt dome and then switch over to a tri cone bit with the projected depth of this well bore section to be 8600' or 50' into the Queenston formation in which then the 9.625 casing will be set. A site walk through review was also conducted with all fluids on site in proper containment. The drilling engineers for XTO and myself spoke briefly over the phone concerning the permanent well head control on the 13.375 casing.
-930233483 2014-05-07 Status check inspection on the completion process found the well has been stimulated and the company is awaiting Halliburton Completion coil tubing unit to arrive and drill out the completing plugs, a site inspection was done and all 500 barrel flow back tanks have proper containment erected and are grounded, 4- 400 barrel production tanks are set up with proper containment.
