2044651 2012-02-22 No Violations Noted Well Id sign and E&S plan were present at access road. At the time of the inspection, no E&S concerns and no freeboard issues at the pit were present at the site. First Class was contracted by NTS. Conductor pipe (20” pipe) was set at 46 feet and was sanded in. No mine shaft or void was encountered. Mine casing (13 3/8” pipe) was set at 295 feet with cement basket at 70’. 270 sacks of Type 1 were used to cement mine casing with 2 bbls returns. Water/mine casing (9 5/8” pipe) was set at 419 feet with cement basket at 210 feet. 150 sks type 1 of cement were used with 3 bbls return. Surface casing (7”) was set at 969 feet with cement basket at 120 feet. 175 sacks type 1 were used with cement to surface. Water was recorded at 10 gpm at 200 feet, 5 gpm at 315 feet and 5 gpm at 800 feet. DEP requested to NTS not to use baskets on cement jobs were good circulation (e.g. 7” and 9” pipe) occurs and requested to move discharge line in pit to avoid a freeboard issue and address access road condition.

At the time of the inspection, no E&S concerns and no freeboard issues at the pit were present at the site. First Class was contracted by NTS. First Class completed well to 3,700 feet. Production pipe was set and Universal was on-site to perform the cementing of pipe.
2046285 2012-02-28 No Violations Noted Well Id sign and E&S plan were present at access road. At the time of the inspection, no E&S concerns and drilling pit was graded and frac pit was present at the site.
2048770 2012-03-06 No Violations Noted Site visit was part of routine inspection due to spud notification.

Access road was extremely steep and unrocked. A bull dozer was pushing water trucks up to site. Spoke to Steve Phillips (NTS) on site. Road is scheduled to be regraded and rocked with clean durable stone. Steep fill slopes were stable. BMPs were in place (haybale backed filter fence) and working properly. Culvert was installed with rock outfall and haybail filter. Well was being frac'ed during time of inspection. A gush of fluids did come from the well head while inspection was being conducted. All other fluids were directed to the pit. No violations noted at this time. PA DEP Water Quality Specialist April Weiland was present during inspection.
