1185622865 2007-07-27 Met with Bill Helmick and Chris Corso both from Whipstock drilling rig at the well stack. The well stake needed to be moved before they install drive pipe. Thewell was moved 1 foot 6 inches from the original well stake. The reason for the move was there needs to be 100 foot distance between the house and the well. There was 100 feet from the stake. The 100 foot distance is from the outside of pipe. Not the center of pipe. Moved the well west 1 foot 6 inches. Drove 20 drive pipe into the ground to 10'. The distance from the house is 101 foot and 6 inches from the house. Dumped 25 bags of cement down around the pipe. Dumped sand around the pipe to surface. Ready for the drilling rig.
1186602330 2007-08-08 I witnessed part of the drilling pits being solidified. I found no violations at this time.
1194437183 2007-11-06 I performed a preliminary restoration status check. Drilling equipment is gone. The debris has been removed. The drilling pits have been solidified and closed. This location is in a baseball parking lot. ID signs posted. There is a fence around the wellhead and the tank battery with locks on. The wellhead has guard rails around it. This well has PASSED PRELIMINARY RESTORATION.
-1597785588 2007-07-31 I witnessed the running and cementing of the 8 5/8 casing. There were no violations at this time
-207098773 2014-08-06 I was on site to conduct a status check with Inspector John Fleming. The site is urban. The wellhead and tank battery are fenced in and locked. A dike surrounds the tank battery that consists of a 140 bbl production tank that is grounded and equipped with a spark arrestor, charcoal filter and a fluid shut off, a 28 bbl brine tank and a vertical separator. The wellhead is equipped with a 1
265062568 2012-07-16 I performed an inspection on the above site and found the tank battery consists of one 140 barrel tank, one 50 barrel tank and one horizontal separator. The well is equipped with a 1 1/2 plunger lift system. Identification signs are posted. No violations at this time.
-371331478 2007-07-31 I performed a rig inspection. Pit liners in good shape. A fence was around entire location. Street drains had straw around them. There was a mud ribbon over the gas line by the street. Location looked good. I found no violations at this time.
-847673461 2007-09-25 I performed a preliminary restoration status check. GPS's both the well and tank battery.Drilling pits closed. Drilling equipment gone. Debris is gone. The location is in a parking lot. Fence around the well. ID sign posted. Fence needs to be placed around the tank battery. This well does not pass preliminary restoration.
