1391615098 2014-02-04 I inspected this well at the request of the Uniontown office to check the idle status of this Berea Sandstone well. The operator has not reported commercial production since 2003. My inspection found the wellhead overgrown with briars and brush. The wellhead consisted of 4.5 casing equipped with a 4 x 2 swedge and ball valve. The valve was closed and there was a 1 plastic line running from the wellhead. A completion report was never submitted by the original owner (Action Petroleum) so casing depths, cement/mud volumes, and completion technique are unknown. The required ID is not posted at the wellhead and there is no associated production or storage equipment. This well is idle and based on previous production records and other Berea wells in the immediate area, does not appear to be capable of producing commercial quantities of oil and/or natural gas. Hall and Horning Oilfield Services, Inc. will be given thirty days to obtain a plugging permit as required by section 1509:13 of the Ohio Revised Code and properly plug and abandon the well in accordance with section 1501:9-11 of the Ohio Administrative Code. Failure to comply with this notice may result in further administrative action including bond forefeiture.
