1759335727 2014-07-31 Inspector Joshua Schiering and I met with landowner Delmar Smith and Attorney Ken Bandy to

conduct a final restoration inspection for the purpose of changing the status of the well from Domestic Well to Final Restoration, since the well was plugged in 1999. The current status listed in the state database indicates the well is currently a domestic Berea well owned by the (Fern) Smith Family Trust. Mrs. Smith is now deceased, and the Smith Family Trust no longer exists. Mr. Smith and Mr. Bandy provided us with paperwork showing that the well has been plugged. Documentation includes an application for a plugging permit signed by Mrs. Smith on August 20, 1999, a copy of the plugging permit issued by ODNR Division of Oil and Gas on September 9, 1999, and a summary sheet compiled by Mr. Smith featuring the dates, activities, and quantities of clay used in plugging the well. The plugging operation was completed on November 11, 1999. According to Mr. Smith, inspector Jonnie Patrick witnessed the plugging. Three other wells owned by the Smith Family Trust were plugged at later dates (61074 plugged on 4/12/2000; 61028 plugged on 9/18/2000; 61515 plugged on 9/23/2001. Inspector Jonnie Patrick approved Final Restoration inspections on all three. Inspector Schiering, Mr. Smith and Mr. Bandy and I went to the site of the well, which is located in a pasture. We reviewed the area of the well location according to Mr. Smith, and also utilized the GPS coordinates for the well obtained from the state database. There was no evidence of the well. Mr. Smith indicated that when the well was plugged, casing was cut off below ground level and they welded a plate on it with the permit number listed.
