152357103 2013-10-28 Well identification illegible. Separator located less than five feet from well head. Well equipped with a one inch steel line from tubing to T fitting, one line runs to separator and one terminates in a nipple with a ball valve. The area around the well head roughly 5x5 feet was scalded. A field test for Chlorides was ran in the mud around the well head and was found to have 1603 ppm Chloride content. No tanks. No lines from separator. A plastic garbage can was near the well and was stained due to salt water. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Mechanical separators shall be set 50 feet from well head. Brine from any well except an exempt Mississippian well shall be disposed of only by injection into an underground formation. All contaminated soils shall be removed and disposed of properly. Any affected areas shall be reseeded and mulched and steps shall be taken to establish vegetative cover.

Waste water was being collected in garbage cans. Allowing brine to evaporate or overflow constitutes an illegal method of storage and disposal. Illegal brine storage, illegal brine disposal, and any further pollution and contamination caused by operation of the well shall cease immediately. Brine from any well shall be disposed of only by injection into an underground formation. Current brine storage and disposal equipment is not rated for expected pressures. All surface equipment must be pressure rated to withstand operating pressures to which it is subjected. Copies of the laws and rules as well as possible penalties provided.
