-465694771 2014-09-08 Asked to check status of well by Columbus. Well equipped with a pumping unit with an electric motor but no power source. Surface and intermediate casing open at surface. Valve on production casing is open and 3/4 plastic line to sales is equipped with a gate valve and a step down regulator. Well appears to be producing house gas. Well is not equipped with a legible identification tag. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. A small amount of fluids have leaked out of the bottom of the tank and contaminated a patch of ground inside the dike. All contaminated materials shall be removed from the dike and disposed of properly. Any leaking equipment shall be repaired or replaced.
